摘 要:目的 比较窄带成像内镜下结直肠病变的毛细血管分型(Sano分型)联合靛胭脂染色检查与普通高清白光内镜检查在结直肠肿瘤性病变中的诊断价值。方法 选取86例结直肠息肉样病变患者(共216个病灶),对其进行窄带成像内镜下Sano分型联合靛胭脂染色检查和普通高清白光内镜检查。将检查结果与病理诊断结果进行比较,分析二者诊断结直肠肿瘤性病变的价值。结果 窄带成像内镜下Sano分型联合靛胭脂染色检查诊断结直肠肿瘤性病变的准确率为97.69%、灵敏度为99.00%、阴性预测值为86.67%,均高于普通高清白光内镜诊断的准确率79.63%、灵敏度81.50%、阴性预测值19.57%(均P<0.05)。结论 与普通高清白光内镜检查相比,窄带成像内镜下Sano分型联合靛胭脂染色检查可提高直肠肿瘤性病变的诊断准确率和灵敏度,降低漏诊率,可为制定临床诊疗方案提供可靠依据,值得在临床中推广使用。Objective To compare the diagnostic value of narrow-band imaging endoscopic Sano classification combined with indigo carmine staining with general high-definition white light endoscopy for colorectal neoplastic lesion.Methods A total of 86 patients with colorectal polypoid lesion(216 lesions in total)were selected,and narrow-band imaging endoscopic Sano classification combined with indigo carmine staining and general high-definition white light endoscopy were used to examine the patients.The results of examinations and pathological diagnosis were compared,and the value of the two methods for diagnosing colorectal neoplastic lesions was analyzed.Results The accuracy,sensitivity,negative predictive value of narrow-band imaging endoscopic Sano classification combined with indigo carmine staining for diagnosing colorectal neoplastic lesion were 97.69%,99.00%,and 86.67%,respectively,which were all higher than those of general high-definition white light endoscopy(79.63%,81.50%,and 19.57%,respectively,all P<0.05).Conclusion Compared with general high-definition white light endoscopy,narrow-band imaging endoscopic Sano classification combined with indigo carmine staining can improve the accuracy and sensitivity for the diagnosis of colorectal neoplastic lesion,decline the rate of missed diagnosis,and provide a reliable basis for the development of clinical diagnosis and treatment regimen,which is worthy of the promotion and application in clinics.
