复杂指向性前交通动脉瘤栓塞治疗附15例临床分析

摘　　要：目的研究介入栓塞治疗复杂指向性前交通动脉瘤的安全性及有效性。方法回顾分析近年来上海交通大学医学院附属瑞金医院(北部院区)行神经介入栓塞治疗15例前交通复杂指向动脉瘤临床资料。结果15例患者均顺利完成栓塞治疗,其实应用支架辅助栓塞12例,双导管栓塞治疗3例。载瘤动脉均保持通畅,随访6~24个月,无再次破裂出血,13例完全恢复,2例患者部分神经功能障碍。一例单纯栓塞患者动脉瘤部分复发再次行支架辅助栓塞治疗,其余患者均未见明显复发。结论介入栓塞治疗复杂指向前交通动脉瘤安全有效,结合支架辅助技术可进一步提高栓塞成功率减少复发率。Objective To study the safety and efficacy of interventional embolization in the treatment of complex directional anterior communicating aneurysm.Methods The clinical data of15cases of anterior communicating complex directional aneurysms treated by interventional embolization in Ruijin North Hospital,Shanghai Jiao Tong University School of Medicine in recent years were retrospectively analyzed.Results All the 15patients completed embolization successfully.In fact,12cases were treated with stent-assisted embolization and 3cases were treated with double catheter embolization.All the parent arteries remained unobstructed,followed up for6-24months,no re-rupture and bleeding,13patients recovered completely,2patients had partial neurological dysfunction.One patient with simple embolization had partial aneurysm recurrence and was treated with stent-assisted embolization again,while no obvious recurrence was observed in the other patients.Conclusion Interventional embolization for complex directional anterior communicating aneurysm is safe and effective,combined with stent-assisted technology can further improve the success rate of embolization and reduce the recurrence rate.
