脑卒中后抑郁状态程度对患者吞咽功能康复效果的影响

摘　　要：目的探讨脑卒中后抑郁状态程度对患者吞咽功能康复效果的影响。方法选取2018年6月至2020年6月本院收治的80例脑卒中患者作为研究对象,根据患者抑郁程度分为轻度抑郁组、中度抑郁组、重度抑郁组、无抑郁组,每组20例。4组均行神经内科脑卒中基础治疗,病情稳定后进行康复训练,比较4组吞咽功能康复情况。结果治疗前,4组吞咽功能评分比较差异无统计学意义;治疗后,4组吞咽功能评分均高于治疗前,且无抑郁组均高于轻度抑郁组、中度抑郁组、重度抑郁组,轻度抑郁组均高于中度抑郁组、重度抑郁组,中度抑郁组高于重度抑郁组,差异均有统计学意义(P<0.05)。结论抑郁状态会影响伴有吞咽障碍脑卒中患者的吞咽功能康复疗效,应及早评估其抑郁程度,进行针对性干预治疗。Objective To investigate the effect of post-stroke depressive state on the recovery of swallowing function in patients.Methods 80 patients with stroke who were admitted to our hospital from June 2018 to June 2020 were selected as the research subjects,they were divided into mild depression group,moderate depression group,severe depression group,and no depression group according to the degree of depression,with 20 cases in each group.All four groups received basic treatment for stroke in neurology department,and rehabilitation training was performed after the condition was stabilized.The recovery of swallowing function of the four groups was compared.Results Before treatment,there was no significant difference in the swallowing function scores among the four groups;after treatment,the swallowing function scores of the four groups were all higher than those before treatment,and the non depression group was higher than the mild depression group,moderate depression group,and severe depression group,the mild depression group was higher than the moderate depression group and the severe depression group,the moderate depression group was higher than the severe depression group,the difference was statistically significant(P<0.05).Conclusion In stroke patients with dysphagia,their own depressive state will affect the recovery effect of swallowing function,the degree of depression should be evaluated early and targeted intervention treatment should be carried out.
