摘　　要：目的:基于DRG指标对医院消化科医疗服务能力进行分析,为医院开展学科评价和推进学科发展提供参考。方法:采集2020年1月1日-2021年12月31日某院消化科出院病案首页数据,经DRG分组器分组,对科室DRG能力评价指标和不同年份间DRG组构成比进行分析。结果:2021年科室总体DRG组数略有增加,CMI值较同期增幅8.05%,总权重较同期增幅27.39%;RW≤0.5的患者占比下降,0.5<RW≤1.0、1.5<RW≤2.0的患者占比上升,外科组患者占比由10.89%上升至26.23%,操作组患者占比由51.71%下降至36.50%,GK25、GK35、GK23病例数占比下降,GT19、GJ15、GJ13病例数占比上升,差异均有统计学意义。结论:通过提高医疗技术,优化病种结构,持续提升病案首页质量,消化科医疗服务能力有较大提升,借助DRG工具可促进学科建设和发展。Objective:To analyze the medical service ability of digestive department in a certain hospital based on DRG index,so as to provide reference for the hospital to carry out discipline evaluation and promote discipline development.Methods:The first page data of discharged medical records from digestive department from January,1,2020 to December,31,2021 were grouped by DRG grouping device to analyze the evaluation indexes of DRG ability and the changes of DRG group composition ratio in different years.Results:In 2021,the number of DRG groups increased slightly,the CMI value increased by 8.05%and the total weight increased by27.39%compared with the same period;The proportion of patients with RW≤0.5 decreased,the proportion of patients with 0.5<RW≤1.0 and 1.5<RW≤2.0 increased,the proportion of patients in surgical group increased from 10.89%to 26.23%,the proportion of patients in operation group decreased from 51.71%to 36.50%,the proportion of GK25,GK35 and GK23 cases decreased,and the proportion of GT19,GJ15 and GJ13 cases increased,differences are statistically significant.Conclusions:By improving medical technology,optimizing disease structure and continuously improving the quality of the first page of medical records,the medical service ability of digestive department were greatly improved.The construction and development of discipline can be promoted with the help of DRG tools.
