摘　　要：多发性骨髓瘤是一种浆细胞恶性增殖的血液系统肿瘤,其典型特征是骨髓的浆细胞异常增殖,继而血清中单克隆免疫球蛋白产生过多,使红细胞表面的负电荷产生变化,形成缗钱状红细胞,从而导致蛋白凝集。近年来,抗-CD38单抗被应用于治疗难治性/复发性多发性骨髓瘤,效果十分显著,但同时发现其会干扰患者血型血清学实验结果。蛋白凝集及抗-CD38单抗的应用是多发性骨髓瘤患者引起临床输血相容性检测困难的主要原因,准确、快速地消除蛋白凝集及抗-CD38单抗在血型鉴定、不规则抗体筛查及交叉配血中的非特异性干扰,是保证多发性骨髓瘤患者临床输血安全的关键。Multiple myeloma is a hematological tumor with malignant proliferation of plasma cells,It is characterized by abnormal proliferation of plasma cells in bone marrow,excessive production of monoclonal immunoglobulin in serum,it changes the negative charge on the surface of red blood cells,pecuniary red blood cells are are taking shape,This leads to protein agglutination.In recent years,CD38 monoclonal antibody is used in the treatment of refractory/recurrent multiple myeloma,The effect is remarkable.At the same time,it was found that CD38 monoclonal antibody interfered with the blood transfusion compatibility testing.protein agglutination and Application of CD38 monoclonal antibody are the main causes of difficulty in blood transfusion compatibility test in patients with multiple myeloma.Accurate and fast eliminate nonspecific interference of protein agglutination and application of CD38 monoclonal antibody in blood group identification、irregular antibody screening and cross matching,It is the key to ensure the safety of clinical blood transfusion in patients with multiple myeloma.
