养阴清热解毒方治疗鼻咽癌急性放射性口腔黏膜炎的临床观察

摘　　要：目的观察养阴清热解毒方治疗鼻咽癌急性放射性口腔黏膜炎(AROM)的临床疗效与安全性。方法将64例鼻咽癌放疗所致AROM患者随机分为观察组与对照组各32例,对照组采取西医常规口腔治疗,观察组在对照组基础上采取养阴清热解毒方治疗。治疗后7 d后观察两组疼痛数字量表(NRS)评分、KPS评分、口腔黏膜损伤分级以及实验室指标,比较两组中医证候疗效。结果治疗后两组NRS评分和KPS评分均较治疗前改善(P<0.05)。观察组治疗后NRS显著低于对照组,KPS评分明显高于对照组(P<0.05)。治疗后两组口腔黏膜损伤分级均有改善(P<0.05),观察组改善程度更明显(P<0.05)。治疗后两组白细胞计数(WBC)、C反应蛋白(CRP)、白细胞介素-6(IL-6)水平均较治疗前下降(P<0.05)。观察组治疗后下降程度更明显(P<0.05)。治疗后观察组总有效率为90.63%,高于对照组的68.75%(P<0.05)。结论养阴清热解毒方联合西医治疗鼻咽癌AROM患者可进一步提高临床疗效,有效缓解患者的疼痛感和口腔黏膜损伤程度,降低WBC、CRP、IL-6水平,改善生活质量。Objective:To investigate the clinical efficacy and safety of Yangyin Qingre Jiedu Decoction in the treatment of acute radiation oral mucositis(AROM)of nasopharyngeal carcinoma.Methods:64 patients with AROM caused by radiotherapy for nasopharyngeal carcinoma were randomly divided into an observation group(n=32)and a control group(n=32).The control group received conventional oral treatment with western medicine,while the observation group received Yangyin Qingre Jiedu Decoction on the basis of the control group.Seven days after treatment,the pain numerical scale(NRS)score,KPS score,oral mucosal injury classification and laboratory indexes were observed in the two groups,and the therapeutic effects of TCM syndromes were compared between the two groups.Results:After treatment,the NRS and KPS scores of the two groups were improved compared with those before treatment(P<0.05).After treatment,the NRS of the observation group was significantly lower than that of the control group,and the KPS score was significantly higher than that of the control group(P<0.05).After treatment,the grades of oral mucosal injury in both groups were improved(P<0.05),and the improvement was more obvious in the observation group(P<0.05).After treatment,the levels of WBC,CRP and IL-6 in the two groups were lower than those before treatment(P<0.05).The degree of decrease in the observation group was more obvious after treatment(P<0.05).After treatment,the total effective rate in the observation group was 90.63%,the total effective rate in the control group was 68.75%,and the total effective rate in the observation group was significantly higher than that in the control group(P<0.05).Conclusion:Yangyin Qingre Jiedu Decoc⁃tion combined with western medicine in the treatment of nasopharyngeal carcinoma patients with AROM can fur⁃ther improve the clinical efficacy,effectively relieve the pain and oral mucosal damage,reduce the levels of WBC,CRP and IL-6,and improve the quality of life.
