不同地域中医名家论治胃食管反流病的学术特点比较

摘　　要：胃食管反流病(GERD)是指胃、十二指肠内容物反流引起不适症状和(或)食管黏膜病理改变的疾病。在全球范围内患病率逐年增加,其病因和发病机制复杂,食管内外均可表现刺激症状,症状隐匿多样,患者常伴有抑郁和(或)焦虑。目前现代医学多用质子泵抑制剂作为主要治疗药物,无明确有效的综合治疗方法。中医流传千年,古籍中已对GERD病名、病因、病机等进行了详尽论述。而因所处年代、地域、环境不同,学术流派之间互相影响,在一代代中医人的思想传承下,现代名中医对GERD的认识、病因病机、治疗用药规律及常用方剂等方面各有见解。现就董建华、徐景藩、李佃贵、劳绍贤、周福生、唐旭东、沈洪等著名医家的经验进行了概述和学术特点比较。以名中医为代表凝练北方、江南、岭南等地医家的学术思想,各位医者学习他们的经验,开阔GERD诊治思路,提高临床水平,在“因人、因时、因地”的思想指导下进行辨证论治。Gastroesophageal reflux disease(GERD)is a clinical condition at which reflux of gastric and duodenal contents causes uncomfortable symptoms and/or pathological changes of esophageal mucosa.The disease presents an increasing incidence year by year and its etiology and pathogenesis are complex.In addition to the irritative symptoms occurring both inside and outside the esophagus,there are various hidden symptoms.The patients are often accompanied by depression and/or anxiety.At present,proton pump inhibitors are mainly used to treat this disease,while clear and effective comprehensive treatment method remains to be developed.Traditional Chinese medicine(TCM)has a history of thousands of years and the etiology and pathogenesis of GERD have been discussed in detail in ancient TCM books.Because of the differences in the time,region,and environment,TCM doctors have developed different academic theories which influence each other.With the inheritance of theories from generations of TCM experts,modern TCM doctors have different opinions about the etiology,pathogenesis,medication rules,and treatment formulae for GERD.This paper summarized and compared the theories of Dong Jianhua,Xu Jingfan,Li Diangui,Lao Shaoxian,Zhou Fusheng,Tang Xudong,and Shen Hong and further condensed the academic thoughts of the experts in north China,south of the Yangtze River,and Lingnan area.It is hoped that all doctors can learn from their experience,broaden the thinking of GERD diagnosis and treatment,and conduct syndrome differentiation and treatment depending on the individual,time,and place.
