IC-VEP联合P-VEP在外伤性视神经病变诊断中的应用研究

摘　　要：目的分析分离格栅视觉诱发电位(IC-VEP)联合图形视觉诱发电位(P-VEP)检测在外伤性视神经病变(TON)诊断中的应用价值。方法回顾性病例研究。选取2019年05月至2021年07月在我院眼科门诊就诊的眼部钝挫伤引起的视力下降患者74例(80只眼),正常组40例(80只眼),所有纳入人员均进行裸眼视力、矫正视力、相干光层析成像术(OCT)、P-VEP、IC-VEP及视野等检查,着重对检测所得的VEP结果数据进行统计分析。结果在对本研究的74例(80只眼)患者进行两种VEP检测结果统计中发现,传统P-VEP在外伤性视神经病变的诊断中敏感性及特异性分别为68.75%、83.75%,其ROC曲线下面积(诊断效能)为0.683;IC-VEP联合P-VEP组检测视神经损伤的敏感性为85%,特异性为81.25%,ROC曲线下面积为0.759。结论IC-VEP联合P-VEP检测在TON诊断中具有较高的敏感性,其诊断效能优于传统P-VEP,减少漏诊,为TON全面诊断提供客观依据。Objective To analyze the application value of isolated-check visual evoked potenyial(IC-VEP)combined with Pattern visual evoked potenyial(P-VEP)in the diagnosis of traumatic optic neuropathy.Methods Totally 80 eyes of 74 vision loss caused by ocular traumacases were selected from the patients treated in our hospital from May 2019 to July 2021 were enrolled,and 80 eyes of 40 normal subjects were selectedas control group.All subjects received OCT,P-VEP,IC-VEP,visual fieldtest,measurement VEP results were analyzed.Results In the statistical results of the two VEP tests performed on 80 eyes of 74 cases in this study,it was found that the sensitivity and specificity of the traditional P-VEP group indiagnose optic nerve injury was 68.75% and 83.75% respectively,the area under the ROC curve(diagnostic efficacy)of it was 0.638 respectively,and the area under the ROC curve ofthe IC-VEP combined with P-VEP groupwas 0.759 respectively(sensitivity 85%,specificity 81.25%).Conclusions JC-VEP combined with P-VEP test has high sensitivity in the diagnosis of traumatic optic nerve injury,and its diagnostic efficacy is better than that traditional P-VEPtest,which can reduce misdiagnosis,provide objective basis for the comprehensive diagnosis of traumatic optic neuropathy,and guide early clinical intervention andtreatment.
