隐匿性眼球破裂CT影像学特征分析

摘　　要：目的分析隐匿性眼球破裂的CT影像学特征。方法横断面研究。分析苏州大学附属第一医院2013年1月至2021年12月单眼眼外伤患者39例(39只眼)的CT影像资料。所有患者就诊时疑似眼球破裂伤行手术探查。观察其影像学特点及临床表现。结果在手术探查中34例确诊眼球破裂伤,其中有23例(67.65%)术前CT提示眼球破裂。隐匿性眼球破裂在CT图像上主要表现为眼球形态改变(85.29%,29/34)和玻璃体内高密度影(70.59%,24/34),与眼球未破裂者CT图像相比,差异具有统计学意义(χ^(2)=10.47,4.85;P=0.007,0.047)。眼球破裂的常见体征是结膜下出血(100.00%,34/34),前房积血(88.24%,30/34)及低眼压(73.53%,25/34)。前房积血和低眼压发生率与眼球未破裂者相比差异有统计学意义(χ^(2)=6.89,5.62;P=0.032,0.035)。结论隐匿性眼球破裂患者行CT检查可为其诊断和治疗提供重要依据。Objective To analyse CT imaging features of patients with occult eyeball rupture.Methods This was a cross-sectional study.The CT imaging data of 39 eyes of 39 patients with monocular trauma from Jan.2013 to Dec.2021 in the First Affiliated Hospital of Soochow University were analyzed.All cases with suspected eye ball rupture received surgical exploration.The imaging and clinical features were observed.Results The eyeball rupture during surgical exploration were diagnosed in all cases in this study.Before surgery,the CT of 23 cases(67.65%)showed eyeball rupture.The CT images of patients with occult ruptured eyeballs were mainly characterized by morphological changes(85.29%,29/34)and intravitreal high-density shadows(70.59%,24/34),which were statistically significant compared with those of unruptured eyeballs(χ^(2)=10.47,4.85;P=0.007,0.047).The common clinical signs of ruptured eyes were conjunctival hematoma(100.00%,34/34),hyphema(88.24%,30/34)and low intraocular pressure(73.53%,25/34),and the difference of the incidence of hyphema and low IOP was statistically significant compared with those of unruptured eyes(χ^(2)=6.89,5.62;P=0.032,0.035).Conclusion CT examination of occult globe rupture can provide important references for its diagnosis and treatment.
