摘 要:目的研究皮下组织连续缝合术在儿童眼周软组织线性切裂伤的疗效。方法回顾性病例对照研究。收集武汉儿童医院2016年7月至2020年1月眼周线性软组织裂伤399例儿童的临床资料。根据治疗方式将患儿分为两组:皮下软组织间断缝合组(200例)与连续缝合组(199例)。随访1个月,比较两组手术时长、术中出血、术后组织肿胀持续时间、医用组织胶有效使用率及术后外观评分。结果连续缝合组手术时长、术中出血及术后组织持续肿胀时间分别为(27.25±6.67)min、(0.84±0.29)ml及(3.40±1.02)d,均明显低于间断缝合组的(31.71±6.71)min、(1.04±0.38)ml及(5.40±1.58)d(t=6.85、7.19及17.10;均P<0.001)。连续缝合组医用组织胶使用189例(94.97%),明显高于间断缝合组的55例(27.50%)(χ^(2)=594.05,P<0.001)。术后1周(1期)及1个月(2期),伤口愈合外观HWES评分6分者,连续缝合组占95.98%(191/199)及98.49%(196/199),均优于间断缝合组的87.00%(174/200)及88.50%(177/200)(χ^(2)=10.39及16.35;均P<0.001)。结论治疗儿童眼周软组织线性切裂伤,皮下组织连续缝合较间断缝合省时、恢复快,而且愈合好。Objective To study the efficacy of continuous subcutaneous suture in children with linear incised wound of periocular soft tissue.Methods A retrospective case-control study was conducted.The clinical data of 399 children with incised wound of periocular soft tissue in Wuhan Children’s Hospital from Jul.2016 to Jan.2020 were collected.The children were divided into two groups with different treatments.There were 200 cases in the interrupted suture group and 199 cases in the continuous suture group.The postoperative follow-up time was 1 month.The operation time,intraoperative bleeding,duration of postoperative tissue swelling,effective utilization rate of medical tissue glue and healing appearance score were compared between the two groups.Results The operation time,intraoperative bleeding and the duration of postoperative tissue swelling were(27.25±6.67)min,(0.84±0.29)ml and(3.40±1.02)d in the continuous suture group,which were significantly lower than the(31.71±6.71)min,(1.04±0.38)ml and(5.40±1.58)d in the interrupted suture group(t=6.85,7.19,17.10;all P<0.001).In the continuous suture group,medical tissue glue was used on 189 cases(94.97%),which was significantly higher than the 55 cases(27.50%)in the interrupted suture group(χ^(2)=594.05,P<0.001).At 1 week(stage 1)and 1 month(stage 2)after operation,the number of cases scored 6 on the wound healing appearance HWES consisted 95.98%(191/199)and 98.49%(196/199)of the continuous suture group.Both were better than the 87.00%(174/200)and 88.50%(177/200)of the interrupted suture group,the differences were statistically significant(χ^(2)=10.39,16.35;all P<0.001).Conclusion The continuous subcutaneous suture for the treatment of children’s linear incised wound of periocular soft tissue uses less time,helps recover faster and improves healing condition,compared with the interrupted subcutaneous suture.
