文眼线所致角膜外伤的治疗效果

摘　　要：目的评价绷带式角膜接触镜联合重组牛碱性成纤维细胞生长因子(rb-bFGF)滴眼液治疗纹眼线术中致角膜外伤的临床效果。方法回顾性病例对照研究。分析郑州市第七人民医院2016年6月至2021年6月文眼线所致角膜外伤140例(224眼)临床资料。将患者分为两组:观察组70例(114眼)和对照组70例(110眼)。观察组为伤后佩戴绷带式角膜接触镜联合rb-bFGF滴眼液,对照组涂rb-bFGF眼用凝胶,治疗后随访1个月。分析两组角膜愈合情况,角膜荧光染色(FL)、泪液分泌试验(Slt)、泪膜破裂时间(BUT)、眼疼痛值改善等效果。结果与治疗前相比,两组患者在治疗后各个时间点的角膜上皮损伤情况、泪液分泌、泪膜破裂时间及疼痛均有明显改善(P<0.001)。并且随访1、3及7 d角膜上皮修复情况观察组明显优于对照组(t=15.93、13.51及11.28,P=0.010、0.020及0.030);SIt两组间差异无统计学意义(t=0.72、0.68、0.72、0.71及0.75,P=0.690、0.722、0.679、0.701及0.631);BUT在治疗后1、3及7 d观察组明显高于对照组(t=0.64、0.73及0.87,P=0.042、0.035及0.028);观察组疼痛值评分在治疗后1、3及7 d明显低于对照组(t=8.13、7.11及6.54,P=0.028、0.041及0.048)。治疗后15及30 d,FL、疼痛值评分、角膜上皮愈合情况、BUT两组差异无统计学意义(P>0.05)。结论绷带式角膜接触镜联合rb-bFGF滴眼液治疗文眼线所致角膜外伤效果良好。Objective To investigate the clinical efficacy of bandage contact lens combined with recombinant bovine basic fibroblast growth factor(rb-bFGF)eye drops in the treatment of corneal injury during tattooing lid-line.Methods This was a retrospective case control study.The clinical data of 224 eyes of 140 patients with corneal injury caused by eyelid line from Jun.2016 to Jun.2021 in the Seventh People’s hospital of Zhengzhou were retrospectively analyzed.These patients were divided into two groups,114 eyes of 70 cases in the observation group and 110 eyes of 70 cases in the control group.The observation group was treated with bandage contact lens combined with rb-bFGF eye drops after injury.The control group was treated with recombinant bovine basic fibrolast growth factor eye-gel.The patients were followed up for 1 months.The corneal healing,corneal fluorescencestaining(FL),Schirmer I test(SIt),tear film break-up time(BUT)and eye pain were recorded and analyzed.Results At each time point after treatment,all parameters of the two groups were significantly improved compared with those before treatment(P<0.001).The repair of corneal epithelium in the observation group was significantly better than that in the control group(t=15.93,13.51,11.28;P=0.010,0.020,0.030).The difference of SIt between the two groups was not statistically significant(t=0.72,0.68,0.72,0.71,0.75;P=0.690,0.722,0.679,0.701,0.631).BUT scores in the observation group were significantly higher than those in the control group(t=0.64,0.73,0.87;P=0.042,0.035,0.028).The pain score of the observation group was significantly lower than that of the control group(t=8.13,7.11,6.54;P=0.028,0.041,0.048).There was no significant difference in FL,pain score,corneal epithelial healing and butscore between the two groups at 15 and 30 days after treatment(P>0.05).Conclusion Bandage contact lens combined with rb-bFGF eye drops is effective,safe and reliable in the treatment of corneal injury caused by tattooing eye lid-line.It canreduce eye symptoms and shorten the
