摘  要:目的 分析结直肠癌患者肠道菌群失调与血清Toll样受体4(TLR4)、血清神经肽Y(NPY)和血管活性肠肽(VIP)水平的相关性。方法 将我院2018年12月-2020年7月收治的87例结直肠癌患者依照肠道菌群检测结果分为肠道菌群正常组、Ⅰ度组、Ⅱ度组;选择同期50例健康体检者作为对照组。检测受试者肠道菌群情况以及血清中TLR4、NPY和VIP水平;采用Pearson相关性检验分析TLR4、NPY和VIP与结直肠癌患者肠道菌群失调的相关性,采用Logistics回归模型分析各指标对结直肠癌患者肠道菌群失调发生发展的影响。结果 不同组别研究对象血清TLR4、NPY和VIP水平存在明显差异,其中对照组中TLR4、NPY和VIP水平最低,Ⅱ度组最高,差异具有统计学意义(P<0.05)。Ⅰ度组、Ⅱ度组真杆菌、双歧杆菌、乳杆菌低于正产组和对照组;而大肠埃希菌、肠球菌均高于正常组和对照组,差异具有统计学意义(P<0.05);同时,Ⅱ度组真杆菌、双歧杆菌、乳杆菌低于Ⅰ度组;而大肠埃希菌、肠球菌均高于Ⅰ度组,差异具有统计学意义(P<0.05)。TLR4、NPY与结直肠癌患者真杆菌、双歧杆菌、乳杆菌呈负相关(P<0.05),与大肠埃希菌、肠球菌成正相关(P<0.05);VIP与结直肠癌患者真杆菌、双歧杆菌、乳杆菌呈正相关(P<0.05),与大肠埃希菌、肠球菌成负相关(P<0.05)。TLR4、NPY和VIP是影响结直肠癌患者肠道菌群失调发生发展的独立性影响因素(P<0.05)。结论 肠道菌群失调与结直肠癌患者血清TLR4、NPY和VIP表达水平存在明显的相关性,且TLR4、NPY和VIP是影响结直肠癌患者肠道菌群失调发生发展的独立性影响因素。Objective To analyze the correlation between intestinal flora imbalance and the levels of serum toll-like receptor 4(TLR4), neuropeptide Y(NPY) and vasoactive intestinal peptide(VIP) in patients with colorectal cancer(CC). Methods Eighty-seven patients with CC admitted to our hospital from December 2018 to July 2020 were divided into normal intestinal flora group, grade Ⅰ group or gradeⅡ group according to the intestinal flora test results;50 healthy individuals in the same period were selected as the control group. The intestinal flora and the levels of TLR4, NPY and VIP in serum were detected;Pearson correlation test was used to analyze the correlation between TLR4, NPY, VIP and intestinal flora disorders. Logistic regression model was used to analyze the impact of each index on the occurrence and development of intestinal flora disorders. Results There were significant differences in the levels of TLR4, NPY and VIP among groups. The levels of TLR4, NPY and VIP in the control group were the lowest and those in the grade Ⅱ group were the highest. The differences were statistically significant(P<0.05). The Eubacterium, Bifidobacteria and Lactobacillus in degree Ⅰ group and degree Ⅱ group were lower, while those of Escherichia coli and Enterococcus were significantly higher than those in the normal group and the control group, respectively(P<0.05). The levels of Eubacterium, Bifidobacterium and Lactobacillus in grade Ⅱ group were lower, while those of E. coli and Enterococcus were significantly higher than those in grade I group, respectively(P<0.05). TLR4 and NPY were negatively correlated with Eubacterium, Bifidobacteria and Lactobacillus(P<0.05), and positively correlated with E. coli and Enterococcus(P<0.05). VIP was positively correlated with Eubacterium, Bifidobacterium and Lactobacillus(P<0.05), and negatively correlated with E. coli and Enterococcus(P<0.05). TLR4, NPY and VIP were independent influencing factors of the occurrence and development of intestinal flora imbalance in patients with CC
