双侧唇红黏膜复合组织瓣推进修复唇红缺损的疗效观察

摘　　要：目的观察双侧唇红黏膜复合组织瓣推进修复下唇1/3~1/2唇红缺损的治疗效果。方法自2019年1月至2022年1月,南昌大学第二附属医院整形美容科收治11例下唇唇红缺损患者,唇红缺损宽度为唇长1/3~1/2,缺损区在红唇中央或偏一侧,缺损区两侧均有正常红唇残留,未累及两侧嘴角。采用双侧唇红黏膜复合组织瓣对缺损进行修复,观察其疗效。结果11例患者唇红黏膜复合组织瓣全部成活,切口均一期愈合,无血肿、切口裂开、感染、复发等并发症发生。所有患者获随访3~12个月,修复后缺损区组织瓣色泽、质地与周边一致,两侧对称,唇部外观满意。切口瘢痕不明显,唇部运动和感觉正常,术后效果满意。结论双侧唇红黏膜复合组织推进皮瓣适合红唇缺损宽度为唇长1/3~1/2,缺损两侧有正常唇组织的患者。该方法操作简单,疗效满意,值得临床推广。Objective To observe the efficacy of bilateral lip vermilion mucosa composite tissue flap in the repair of 1/3 to 1/2 lip vermilion defects of lower lip.Methods Eleven patients with lip vermilion defects of lower lip in our department from January 2019 to January 2022 were collected.The width of lip vermilion defects was 1/3 to 1/2 length of lip.The defect was in the center or one side of red lip.Normal red lip was left on both sides of defect,without involving corners of the mouth on both sides.The defects were treated with bilateral lip vermilion mucosa composite tissue flap and the efficacy was observed.Results All lip vermilion mucosa composite tissue flaps in 11 patients survived,and all incisions healed by first intension.No complications such as hematoma,wound dehiscence,infection and recurrence occurred.After 3 to 12 months of follow-up,the color and texture of lip vermilion mucosa composite tissue flap in the defect were consistent with surrounding area.The symmetry and appearance of lip was satisfactory.The scar in incision was not obvious,and the movement and sensation of lip were normal.Patients were satisfied with the postoperative efficacy.Conclusion Bilateral lip vermilion mucosa composite tissue flap is suitable for patients with defects ranging from 1/3 to 1/2 length of lip and normal lip remained on both sides of defects.This method is simple and effective.It should be considerable for clinical promotion.
