摘  要:目的 探讨幽门螺杆菌(Hp)感染基因型与胃肠疾病的关系。方法 选取2017年6月-2020年6月新乡市中心医院258例胃肠疾病患者,其中慢性胃炎患者112例(慢性胃炎组)、消化性溃疡患者98例(消化性溃疡组)、胃癌患者48例(胃癌组),聚合酶链式反应(PCR)检测Hp基因型并分析其在胃肠疾病中的分布情况,分析Hp感染基因型与胃肠疾病患者预后关系。结果 消化性溃疡组胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)、胃泌素-17(G-17)、降钙素原(PCT)、C-反应蛋白(CRP)、中性粒细胞百分比、淋巴细胞百分比、单核细胞百分比均高于慢性胃炎组(P<0.05),胃癌组PGⅠ、PGⅡ和中性粒细胞百分比水平均低于慢性胃炎组(P<0.05),胃癌组G-17高于慢性胃炎组(P<0.05);胃癌组PGⅠ、PGⅡ、CRP水平和中性粒细胞百分比、淋巴细胞百分比、单核细胞百分比均低于消化性溃疡组(P<0.05),而G-17水平高于消化性溃疡组(P<0.05);消化性溃疡组Hp感染基因型cagA、iceA、baba2占比低于慢性胃炎组(P<0.05),而vacA占比高于慢性胃炎组(P<0.05);胃癌组vacA占比高于慢性胃炎组(P<0.05),而iceA占比低于慢性胃炎组和消化性溃疡组(P<0.05);胃癌预后差患者中vacA基因亚型m2占比高于慢性胃炎组和消化性溃疡组(P<0.05)。结论 Hp感染基因型可能影响胃肠疾病病变程度,其中Hp感染基因型vacA亚型m2可能与胃癌不良预后存在关系。OBJECTIVE To explore the relationship between genotypes of Helicobacter pylori(Hp) infection and gastrointestinal diseases. METHODS A total of 258 patients with gastrointestinal diseases in Xinxiang Central Hospital between Jun 2017 and Jun 2020 were enrolled, including 112 cases with chronic gastritis(chronic gastritis group), 98 cases with peptic ulcer(peptic ulcer group) and 48 cases with gastric cancer(gastric cancer group). Hp genotypes were detected by polymerase chain reaction(PCR), and their distribution in gastrointestinal diseases was detected. The relationship between genotypes of Hp infection and prognosis in patients with gastrointestinal diseases was analyzed. RESULTS The pepsinogen I(PG I), pepsinogen II(PG II), gastrin-17(G-17), procalcitonin(PCT), C-reactive protein(CRP), percentages of neutrophils, lymphocytes and monocytes in the peptic ulcer group were significantly higher than those in the chronic gastritis group(P<0.05). PG I, PG II and percentage of neutrophils in the gastric cancer group were significantly lower than those in the chronic gastritis group(P<0.05), while G-17 level was significantly higher than that in the chronic gastritis group(P<0.05). The levels of PG I, PG II and CRP, and the percentages of neutrophils, lymphocytes and monocytes in the gastric cancer group were significantly lower than those in the peptic ulcer group(P<0.05), while G-17 level was significantly higher than that in the peptic ulcer group(P<0.05). The ratios of Hp infection genotypes(cagA, iceA, baba2) in the peptic ulcer group were significantly lower than those in the chronic gastritis group(P<0.05), while ratio of vacA genotype was significantly higher than that in the chronic gastritis group(P<0.05). The ratio of vacA in the gastric cancer group was significantly higher than that in the chronic gastritis group(P<0.05), while ratio of iceA was significantly lower than that in the chronic gastritis group and peptic ulcer group(P<0.05).
