摘 要:目的 分析表面活性蛋白A(SP-A)和表面活性蛋白D(SP-D)基因多态性及尿液中含量与膀胱癌术后尿路感染的关联性。方法 选择2019年1月-2020年1月于新乡医学院第一附属医院行经尿道膀胱肿瘤切除术的203例膀胱癌患者,分为术后尿路感染组(n=38)和无尿路感染组(n=165),采用聚合酶链反应-序列特异性引物(PCR-SSP)分析SP-A及SP-D基因位点单核苷酸多态性(SNPs),比较两组尿液中SP-A及SP-D含量。结果 尿路感染组SP-A1基因Ala19Val位点Val/Val基因型频率、19Val等位基因频率分别为15.79%、48.68%, SP-A2基因Lys223Gln位点Lys/Lys基因型频率、223Lys等位基因频率分别为5.26%、14.47%, SP-D基因Thr11Met位点Met/Met基因型频率、11Met等位基因频率分别为26.32%、53.95%,均低于无尿路感染组(P<0.05);尿路感染组尿液中SP-A及SP-D含量均低于无尿路感染组(P<0.05)。结论 SP-A1基因Ala19Val位点19Val等位基因、SP-A2基因Lys223Gln位点223Gln等位基因以及SP-D基因Thr11Met位点11Met等位基因突变,以及尿液中SP-A及SP-D含量降低可能与膀胱癌术后尿路感染存在关联性。OBJECTIVE To observe the association between the gene polymorphisms of surfactant protein A(SP-A) and surfactant protein D(SP-D), their levels in urine and the postoperative urinary tract infection in bladder cancer patients. METHODS Totally 203 bladder cancer patients who underwent transurethral resection of bladder tumor in the First Affiliated Hospital of Zhengzhou University from Jan 2019 to Jan 2020 were enrolled in the study and divided into the urinary tract infection group with 38 cases and the no urinary tract infection group with 165 cases. The single nucleotide polymorphisms(SNPs) at the loci of SP-A and SP-D genes were analyzed by means of polymerase chain reaction-sequence specific primers(PCR-SSP), and the levels of urine SP-A and SP-D were compared between the two groups. RESULTS The frequencies of Val/Val genotype and 19 Val allele at Ala19 Val locus of SP-A1 gene were respectively 15.79% and 48.68% in the urinary tract infection group;the frequencies of Lys/Lys genotype and 223 Lys allele at Lys223 Gln locus of SP-A2 gene were 5.26% and 14.47%, respectively;the frequencies of Met/Met genotype and 11 Met allele at Thr11 Met locus of SP-D gene were 26.32% and 53.95%, respectively;the frequencies of all the genotypes and alleles were significantly lower in the urinary tract infection than in the no urinary tract infection group(P<0.05). CONCLUSION There are mutations of allele 19 Val at Ala19 Val locus of SP-A1 gene, allele 223 Gln at Lys223 Gln locus of SP-A2 gene and allele 11 Met at Thr11 Met locus of SP-D gene, the decline of urine SP-A and SP-D levels may be associated with the postoperative urinary tract infection in the bladder cancer patients.
