摘 要:目的探究自动化合成^(18)F-AlF-1,4,7-三氮杂环壬烷-1,4,7-三乙酸(NOTA)-奥曲肽的方法并进行神经内分泌肿瘤(NET)显像。方法利用Trasis AllinOne合成模块,将Al^(18)F与前体NOTA-奥曲肽在100℃下通过鳌合反应自动化合成^(18)F-AlF-NOTA-奥曲肽,并对产品进行质量分析。对1例NET患者(男,35岁)行^(18)F-AlF-NOTA-奥曲肽和^(18)F-FDG PET/CT显像并比较显像结果。结果^(18)F-AlF-NOTA-奥曲肽总合成时间为35 min,合成产率为(55.8±1.8)%(经非衰减校正,n=6),放化纯>95%,稳定性好,产品的无菌和无热原要求均符合规定。与^(18)F-FDG相比(病灶SUVmax为3.8,靶/本底比值为1.03),^(18)F-AlF-NOTA-奥曲肽在NET患者体内显像清楚,病灶SUVmax为21.7,靶/本底比值为4.09。结论利用Trasis AllinOne合成模块制备^(18)F-AlF-NOTA-奥曲肽简单快捷,方法稳定,得到的产品放化纯高,能满足临床应用需求。^(18)F-AlF-NOTA-奥曲肽在NET患者体内显像具有较高的靶/本底比值,与^(18)F-FDG相比有明显的优势。Objective To explore the automatic synthesis method of ^(18)F-AlF-1,4,7-triazacyclononane-1,4,7-triacetic acid(NOTA)-octreotide and the imaging in neuroendocrine tumor(NET).Methods Using the Trasis AllinOne synthesis module,^(18)F-AlF-NOTA-octreotide was automatically synthesized by chelating ^(18)F-AlF and NOTA-octreotide at 100℃,and the quality of the product was analyzed.^(18)F-AlF-NOTA-octreotide and ^(18)F-FDG PET/CT imaging were performed and compared in one patient with NET(male,35 years old).Results The total synthesis time of ^(18)F-AlF-NOTA-octreotide was 35 min with a radiochemical yield of(55.8±1.8%)%(non-decay corrected,n=6),radiochemical purity more than 95%and good stability.The sterility and pyrogen-free of the product met the requirements.Compared with ^(18)F-FDG(the lesion SUVmax=3.8,target-to-background(T/B)ratio=1.03),^(18)F-AlF-NOTA-octreotide could be clearly imaged in the patient with NET with SUVmax of 21.7 and T/B ratio of 4.09.Conclusions The preparation of ^(18)F-AlF-NOTA-octreotide with Trasis Allinone synthesis module is simple,rapid and stable.The product has high radiochemical purity and can meet the needs of clinical application.In vivo^(18)F-ALF-NOTA-octreotide PET/CT imaging in the patient with NET shows higher T/B ratio,which has obvious advantages compared with ^(18)F-FDG.
