乳腺癌远处转移相关因素的回顾性研究

摘　　要：目的探寻乳腺癌远处转移及其影响因素,为转移性乳腺癌治疗提供理论依据。方法回顾性分析2015年1月至2017年12月在首都医科大学附属北京天坛医院乳腺科进行手术的204例乳腺癌患者远处转移情况及其影响因素。结果25例乳腺癌患者发生远处转移(12.3%)。乳腺癌远处转移与肿瘤大小、有无腋窝淋巴结转移及有无脉管癌栓相关(P<0.05),进一步研究分析远处转移与腋窝淋巴结转移个数相关(P<0.05)。但与年龄、是否绝经、乳腺癌卵巢癌家族史、肿瘤分级、雌激素受体、孕激素受体、人类表皮生长因子受体2、三阴性乳腺癌无关(P>0.05)。结论肿瘤大小、腋窝淋巴结转移及脉管癌栓与乳腺癌远处转移密切相关。Objective To analyze distant metastasis of breast cancer and its influencing factors.Methods Chi-square test or Fisher′s exact probability test were used to analyze the association between distant metastasis of breast cancer and its influencing factors in invasive ductal breast cancer patients diagnosed in the department of breast disease of Beijing Tiantan Hospital,Capital Medical University from January 2015 to December 2017.Results The percentage of distant metastasis of breast cancer was 12.3%.Distant metastasis was associated with larger tumor size,axillary lymph node metastasis,and vascular tumor thrombus(P<0.05),and patients with more axillary lymph nodes metastasis had a higher risk of distant metastasis(P<0.05).However,no association between distant metastasis and age at diagnosis,menstrual status,grade,stage,estrogen receptor,progesterone receptor and human epidermal factor receptor 2 status,and triple negative breast cancer(P>0.05).Conclusions Tumor size,axillary lymph node metastasis,and vascular tumor thrombus play a pivotal role in distant metastasis of breast cancer.
