牙周牙髓联合病变患者采用冷牙胶侧方加压与热牙胶垂直加压根管充填治疗的效果对比研究

摘　　要：目的:比较冷牙胶侧方加压与热牙胶垂直加压治疗牙周牙髓联合病变的效果。方法:选择2019年2月-2021年2月台山市人民医院收治的66例牙周牙髓联合病变患者作为研究对象。按照治疗方案将其分为对照组和试验组,每组33例。对照组采用冷牙胶侧方加压根管充填治疗方案,试验组采用热牙胶垂直加压根管充填治疗方案。比较两组治疗效果、充填效果、疼痛程度、牙周指数。结果:试验组治疗总有效率、充填优良率分别为90.91%、96.97%,均明显高于对照组的69.70%、81.82%,差异均有统计学意义(P<0.05)。治疗前,两组VAS评分、牙周指数比较,差异均无统计学意义(P>0.05);治疗后,两组VAS评分、牙龈指数、出血指数、菌斑指数较治疗前均降低,且试验组低于对照组(P<0.05)。结论:热牙胶垂直加压根管充填治疗方案治疗牙周牙髓联合病变的效果好于冷牙胶侧方加压,应作为首选治疗方案。Objective:To compare the effect of lateral compression of cold gutta-percha and vertical compression of hot gutta-percha for root canal filling in periodontal and endodontic lesions.Method:A total of 66 patients with combined periodontal and endodontic lesions admitted to Taishan People’s Hospital from February 2019 to February 2021 were selected as the research objects.According to the treatment plan,they were divided into control group and experimental group,33 cases in each group.The control group was treated with lateral compression of cold tooth glue,while the experimental group was treated with vertical compression of hot tooth glue.The treatment effect,filling effect,pain degree,periodontal index were compared between the two groups.Result:The total effective rate and excellent filling rate of the experimental group were 90.91%and 96.97%respectively,which were significantly higher than 69.70%and 81.82%of the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in VAS score and periodontal index between two groups(P>0.05);after treatment,the VAS score,gingival index,bleeding index and plaque index of two groups were lower than those before treatment,and the experimental group were lower than those of the control group(P<0.05).Conclusion:The effect of hot gutta-percha vertical compression root canal filling is better than that of cold gutta-percha lateral compression in the treatment of periodontal and pulpal combined lesions,and it should be the first choice for treatment.
