摘　　要：目的:探讨腹腔镜机器人“3+2”模式应用于远端胃癌根治术中的经济性及安全性。方法:回顾性分析20例施行腹腔镜机器人“3+2”模式远端胃癌根治术(3+2组)及20例接受传统腹腔镜机器人手术(对照组)的患者围术期资料,分别对两组术中情况、术后临床指标、术后机体血清应激指标进行比较。结果:两组患者淋巴结清扫数量及术后早期并发症数量差异均无统计学意义(P>0.05)。3+2组手术时间及出血量均少于对照组,差异有统计学意义(P<0.05)。3+2组患者在术后第1、3天视觉疼痛评分、白细胞、C-反应蛋白、手术总花费、住院总花费及住院时间方面水平均低于对照组,差异均有统计学意义(P<0.05)。结论:腹腔镜机器人“3+2”模式在远端胃癌根治术的临床应用中具有术后疼痛更轻、术后炎性反应更轻、花费更少及术后住院时间更短等优势,具有较好的经济性和安全性。Objective:The economy and safety of laparoscopic robot"3+2"mode(3+2)in radical gastrectomy for distal gastric cancer.Methods:Using the method of retrospective analysis,20patients who underwent laparoscopic robot"3+2"mode radical gastrectomy for distal gastric cancer(3+2group)and 20 patients who underwent traditional laparoscopic robotic surgery in radical gastrectomy for distal gastric cancer(control group)were selected.The intraoperative conditions,postoperative clinical indexes and postoperative serum stress indexes of the two groups were observed and statistically analyzed.Results:There was no significant difference in the number of lymph node dissections and early postoperative complications between the two groups(P>0.05).The operation time and bleeding volume in group 3+2 were less than those in the control group(P<0.05).The levels of visual pain score,leukocyte,C-reactive protein,total operation cost,total hospitalization cost and hospitalization time in 3+2 group were lower than those in CLS group(P<0.05).Conclusion:In the clinical application of laparoscopic robot"3+2"mode in radical gastrectomy for distal gastric cancer,it has the advantages of lighter postoperative pain,lighter postoperative inflammatory reaction,less operation,less hospitalization cost and shorter postoperative hospitalization time.It has good economy and safety.
