摘 要:目的分析艾芬地尔(Ifenprodil)对脑缺血再灌注大鼠脑细胞NR2B亚基表达的影响及其对脑的保护效应。方法选取清洁级雄性SD大鼠67只,经由线栓法完成大脑中动脉缺血再灌注损伤模型的构建。采用随机数字表法将大鼠分为假手术组、艾芬地尔1 h组、艾芬地尔2 h组、艾芬地尔3 h组、对照1 h组、对照2 h组、对照3 h组。除假手术组7只大鼠外,剩余每组各10只大鼠。艾芬地尔各组动物再灌注0.5 h前腹腔注入2 mg/kg艾芬地尔,对照组注入等量生理盐水,假手术组不进行处理。脑血流再灌注1 d后,评估大鼠神经功能缺损程度,采用2,3,5-三苯基氯化四氮唑(TTC)染色测定脑梗死体积,采用免疫组织化学染色(IHC)法对脑细胞内NR2B亚基表达进行测定。结果艾芬地尔1 h组神经功能缺损评分低于对照1 h组,艾芬地尔2 h组神经功能缺损评分低于对照2 h组,差异均有统计学意义(P<0.05)。艾芬地尔1 h组脑梗死体积小于对照1 h组,艾芬地尔2 h组脑梗死体积小于对照2 h组,差异均有统计学意义(P<0.05)。各组动物额叶皮层细胞浆、细胞膜中均可见NR2B亚基表达。艾芬地尔1 h组、艾芬地尔2 h组、艾芬地尔3 h组NR2B亚基表达呈上升趋势(P<0.05);对照1 h组、对照2 h组、对照3 h组NR2B亚基表达呈上升趋势(P<0.05)。与假手术组比较,对照1 h组与艾芬地尔1 h组NR2B亚基表达水平较低(P<0.01);对照3 h组NR2B亚基表达水平较高(P<0.05)。艾芬地尔1 h组NR2B亚基表达水平低于对照1 h组,艾芬地尔2 h组NR2B亚基表达水平低于对照2 h组、艾芬地尔3 h组NR2B亚基表达水平低于对照3 h组,差异均有统计学意义(P<0.05)。结论艾芬地尔可改善缺血再灌注损伤,并可下调NR2B亚基表达,由此削弱此亚基介导的兴奋性氨基酸毒性效应,发挥神经保护作用。Objective To analyze the effect of Ifenprodil on the expression of NR2 B subunit in cerebral ischemia-reperfusion rat brain cells and its protective effect on brain.Methods A total of 67 clean grade male Sprague-Dawley(SD)rats were selected and the middle cerebral artery ischemia-reperfusion injury model was established by suture method.The rats were divided into sham operation group,Ifenprodil 1 h group,Ifenprodil 2 h group,Ifenprodil 3 h group,control 1 h group,control 2 h group,and control 3 h group by random number method.There were 10 rats in each group except 7 rats in the sham operation group.The animals in the Ifenprodil group were intraperitoneally injected with 2 mg/kg Ifenprodil before 0.5 h of reperfusion,and the animals in the control group were injected with the same amount of normal saline,while the animals in the sham operation group were not treated.One day after cerebral blood flow reperfusion,the neurological deficit of rats was evaluated.The infarct volume was determined by 2,3,5-triphenyltetrazolium chloride(TTC)staining,and the expression of NR2 B subunit in brain cells was determined by immunohistochemical staining(IHC).Results Ifenprodil 1 h group showed lower neurological deficit score compared with that in the control 1 h group,and Ifenprodil 2 h group showed lower neurological deficit score compared with that in the control 2 h group(P<0.05).Ifenprodil 1 h group showed smaller cerebral infarct volume compared with that in the control 1 h group,and Ifenprodil 2 h group showed smaller cerebral infarct volume compared with that in the control 2 h group(P<0.05).The expression of NR2 B subunit was observed in the cytoplasm and cell membrane of frontal cortex in all groups.The expression of NR2 B subunits in Ifenprodil 1 h,Ifenprodil 2 h,and Ifenprodil 3 h groups showed an upward trend(P<0.05).The expression of NR2 B subunit in control 1 h group,control 2 h group,and control 3 h group showed an upward trend(P<0.05).Compared with the sham operation group,the expression level of NR2 B subunit i
