颊肌黏膜瓣在口腔癌术后软组织缺损修复中的应用

摘　　要：目的:探讨颊肌黏膜瓣在口腔癌术后软组织缺损修复中的应用价值。方法:连续性纳入2020年1月-2021年1月笔者医院收治的26例口腔癌患者,均进行病灶扩大切除术,术后根据缺损部位分别选择面动脉颊肌黏膜瓣(FAMM)、颊动脉颊肌黏膜瓣(BAMM)及面动脉岛状颊肌黏膜瓣(FAMMIF)进行修复重建,观察皮瓣存活率、修复手术时间、住院时间、修复效果及术后并发症情况。结果:26例患者中,8例采用FAMM修复,9例采用BAMM修复,9例采用FAMMIF修复。皮瓣成活率为88.46%(23/26),均为Ⅰ期愈合,修复手术时间为(3.04±0.52)h,住院时间为(13.27±2.20)d。修复后皮肤色泽评分为(8.48±0.55)分,创面美观评分为(8.60±0.79)分,颌面功能评分为(7.45±0.35)分。未发生血管危象、供区血肿及感染等术后并发症。结论:颊肌黏膜瓣对口腔癌术后软组织缺损的修复效果显著,皮瓣成活率高,安全性好。Objective To investigate the application value of buccal myomucosal flap in the repair of soft tissue defect after surgery for oral cancer.Methods A total of 26 patients with oral cancer who were successively included in our hospital from January 2020 to January 2021.All the patients were carried out enlarged lesion resection.After surgery,according to the defect site,the facial artery myomucosal flap(FAMM),buccal artery myomucosal flap(BAMM)and facial artery myomucosal island flap(FAMMIF)was selected for repair and reconstruction.The survival rate of skin flap,repair operation time,hospital stays,repair effect and postoperative complications were observed.Results Among the 26 patients,8 cases were repaired by FAMM,9 cases were repaired by BAMM,9 cases were repaired by FAMMIF.The survival rate of skin flap was 88.46%(23/26),all areⅠhealing.The repair operation time was(3.04±0.52)h and the hospital stays were(13.27±2.20)d.After repair,the skin color scores were(8.48±0.55)points,the wound aesthetic scores were(8.60±0.79)points and the maxillofacial function scores were(7.45±0.35)points.There were no postoperative complications such as vascular crisis,donor hematoma,infection and so on.Conclusion The repair effect was remarkable of buccal myomucosal flap on soft tissue defect after surgery for oral cancer,the survival rate of skin flap was high and the safety was good.
