玻璃纤维桩修复牙体缺损患者的综合效果分析

摘　　要：目的:探讨玻璃纤维桩修复牙体缺损的综合效果,并观察患者对修复效果的满意度及对牙周碱性磷酸酶(Alkaline phosphatase protein,ALP)水平的影响。方法:将89例牙体缺损修复患者纳为研究对象,根据修复桩核类型,将其分为观察组(玻璃纤维桩修复,n=47,共59颗牙)与对照组(铸造钴铬合金桩修复,n=42,共50颗牙),随访2年,比较两组修复效果、牙周碱性磷酸酶水平及患者修复满意度。结果:观察组桩核修复成功率为98.31%,显著高于对照组的86.00%,差异有统计学意义(P<0.05);两组修复失败类型,以及修复体完整度、舒适度、边缘密合度及边缘着色情况比较,差异无统计学意义(P>0.05)。修复后2年,观察组牙周ALP水平与其修复前比较,差异无统计学意义(P>0.05);对照组修复后2年牙周ALP水平较其修复前显著升高,且显著高于观察组修复后2年水平,差异均有统计学意义(P<0.05)。修复后2年,观察组牙周袋硫化物阳性率显著低于对照组(P<0.05);修复后2年,两组牙周指标包括BI、PLI以及PD水平均较同组修复前显著下降,而且对照组BI、PLI以及PD水平均显著低于观察组,差异均有统计学意义(P<0.05);观察组修复满意度为93.62%,显著高于对照组的78.57%,差异有统计学意义(P<0.05)。结论:与铸造钴铬合金桩相比,玻璃纤维桩在修复牙体缺损中具有更好的实用性,其修复成功率更高,可提高患者修复满意度。Objective To analyze the comprehensive effect of glass fiber posts in the restoration of dental defects,and to analyze its effect on patient satisfaction and periodontal alkaline phosphatase(ALP)level.Methods Eighty-nine patients with dental defects underwent restoration in our hospital were enrolled,and classified into two groups according to different types of posts.A sample of 59 teeth(n=47)restored with glass fiber post were set as observation group,and another 50 teeth(n=42)restored with Ni-Cr alloy post were set as control group.All patients completed a two-year follow-up.Then the restoration effect,periodontal ALP level and patient satisfaction were compared between groups.Results The successful repair rate was 98.31%in observation group,which was significantly higher than 86.00%in control group(P<0.05),while no statistic difference was found in the failure rate between two groups(P>0.05).The integrity,comfort degree,edge fitness and staining of dental prosthetics demonstrated no significant difference between two groups(P>0.05).Two years after restoration,periodontal ALP level showed no significant change in observation group(P>0.05),while was significantly increased in control group(P<0.05),so the periodontal ALP level of control group was significantly higher than that of observation group at the end of follow-up(P<0.05),meantime,the positive rate of sulfide in periodontal pockets in observation group was significantly lower than that in control group(P<0.05).Compared with baseline data,the periodontal BI,PLI and PD values were significantly decreased in both groups two years after repair(P<0.05),and the values in control group were significantly lower than those in observation group,the differences were statistically significant(P<0.05).The restoration satisfaction rate was 93.62%in observation group,which was remarkably higher than 78.57%in control group,the differences was statistically significant(P<0.05).Conclusion Compared with Ni-Cr alloy post,glass fiber post in dental restoration has better mec
