摘　　要：目的:探究玻璃纤维桩氧化锆全瓷冠在磨牙大面积缺损修复中的应用及美学效果。方法:选择2018年1月-2020年5月笔者科室收治的80例磨牙大面积缺损患者为研究对象,根据修复方式不同分为观察组42例(48颗患牙)和对照组38例(43颗患牙),分别以玻璃纤维桩氧化锆全瓷冠、金属桩钴铬烤瓷冠修复。随访1年,比较两组修复成功率、修复效果、炎症因子、咀嚼效率及美学评分。结果:修复1年后,观察组修复成功率为93.75%,稍高于对照组的86.05%,两组比较差异无统计学意义(P>0.05);修复1年后,观察组的修复体完整度、颜色匹配、边缘适合性、边缘着色、牙龈状况的合格率均显著高于对照组,观察组修复后1个月后的白细胞介素-6(IL-6)、IL-8和肿瘤坏死因子(TNF-α)水平均显著低于对照组,观察组修复1个月后咀嚼效率、主观和客观美学评分均显著高于对照组,差异均有统计学意义(均P<0.05)。结论:玻璃纤维桩氧化锆全瓷冠在磨牙大面积缺损修复中可提高成功率和美学效果,有利于减轻牙龈组织炎症、恢复咀嚼功能。Objective To explore the application and cosmetic effect of glass fiber post zirconia all-ceramic crowns in the restoration of molar teeth with extensive defect.Methods 80 patients with extensive defect in molar teeth who were treated between January 2018 and May 2020 were selected as the research subjects.According to different restoration methods,the patients were divided into observation group(42 cases,48 teeth)and control group(38 cases,43 teeth).The two groups were given metal post cobalt chromium ceramic crowns and glass fiber post zirconia all-ceramic crowns for restoration respectively.The success rate of repair,repair effect,inflammatory factors,masticatory efficiency and aesthetic score were compared between the two groups after 1 year of follow-up.Results The restoration success rate of observation group was slightly higher than that of control group(93.75%vs 86.05%)at 1 year after restoration(P>0.05).The qualification rates of restoration integrity,color matching,edge fitness,edge coloration and gingival condition were significantly higher in observation group than those in control group after restoration(P<0.05).The levels of interleukin-6(IL-6),IL-8 and tumor necrosis factor(TNF-ɑ)in observation group at 1 month after restoration were significantly lower than those in control group(P<0.05).The scores of masticatory efficiency,subjective and objective aesthetics of observation group were significantly higher than those of control group after 1 month of restoration(P<0.05).Conclusion Glass fiber post zirconia all-ceramic crowns can improve the success rate and aesthetic effect in the restoration of molar teeth with extensive defect,and is beneficial to reducing the gingival tissue inflammation and restoring the ideal masticatory function.
