骨性Ⅲ类均角错牙合牙颌特征及其与下颌骨位置相关性分析

摘  要:目的:了解均角骨性Ⅲ类牙齿生理性初始位全牙轴倾斜特征及其与下颌骨矢/垂直向位置的相关性。方法:收集均角骨性Ⅰ类30例、Ⅲ类21例患者CBCT Dicom数据,导入Invivo 5.1软件测量相关指标。结果:均角Ⅲ类和Ⅰ类同颌同名牙转矩角和轴倾角一致性较好。Ⅲ类与Ⅰ类相比:①矢状向:上切牙段冠唇向、下切牙段冠舌向趋势增大,差异有统计学意义(P<0.05);上下尖牙、后牙轴倾角比较,差异无统计学意义;②冠状向:上切牙近中倾斜趋势增加,下切牙较直立,仅上颌侧切牙比较差异有统计学意义(P<0.05);上尖牙及后牙冠颊向、下尖牙及后牙冠舌向趋势增大,仅上前磨牙比较差异有统计学意义(P<0.05);③均角Ⅲ类MP-FH值与L5、L6、L7转矩角正相关,且牙位越靠后相关性越强;④下前牙转矩值与ANB值正相关。结论:①骨性Ⅲ类均角牙列初始位时,下前牙转矩随ANB角减小而舌倾趋势增加,下颌后牙随MP-FH增大而呈颊倾趋势,上牙列与ANB角和MP-FH间无明显的相关性;②骨性Ⅲ类均角下牙列牙齿呈舌向倾斜,下牙列舌向安全代偿范围相对减小。Objective To analyze the characteristics of the initial whole teeth inclination and the correlation between the inclination and position of mandible in saggital and vertical direction.Methods Including the skeletal class I andⅢmalocclusion patients’dicom data of CBCT were 30 and 21 respectively with normal-angle were collected.Tip and torque of whole teeth were measured separately by the software Invivo5.1.Results The bilateral homonymous teeth inclination in same jaw had a good consistency in normal-angle skeletal Class I andⅢ.Compared the skeletal ClassⅢmalocclusion with the Class I malocclusion:①saggital view:the trend of crown labial torque increased in the upper incisors and decreased in the lower incisors,the difference was statistically significant(P<0.05).Tip of canines and posteriors in upper and lower jaw showed no statistically significant difference.②Coronal view:maxillary incisors and lateral incisors inclined mesially,and the madibular incisiors were upright,however the statistically significant difference were found only in laeral incisiors,the difference was statistically significant(P<0.05).Crown torque of the upper canines and posteriors were buccally inclined and the lower were lingually inclined increasingly,only the maxillary premolars showed a significant difference.③A significantly postive correlation were noted between angle of MP-FH and the torque value of L5,L6 and L7.④A significantly postive correction were noted between the torque value of lower anteriors and value of ANB.Conclusion①At the initial position:the anterior tooth torque increased with the increase of ANB Angle and the trend of lip inclination increased;The mandibular posterior teeth at the initial position showed buccal inclination with the increase of MP-FH.There was no significant correlation between the inclination of upper dentition and ANB Angle and MP-FH.②The inferior dentition were lingually inclined in the normal-angle skeletal classⅢmalocclusion,so the safe lingual compensatory treatment range
