摘　　要：糖尿病足溃疡是糖尿病最常见、最严重的并发症之一,是糖尿病患者致残、致死的主要原因之一。糖尿病足溃疡常经久难愈,中医治疗本病有其独特优势,现在医家多认为糖尿病足溃疡属中医学脱疽、筋疽范畴,为本虚标实之证,湿热毒瘀是其主要病理因素,病变脏腑涉及肝脾肾等;辨证当局部辨证与整体辨证结合,以局部辨证为主;治疗则分期分型论治,内外合治。现代中医学对糖尿病足溃疡的认识有了新的发展,但不同流派及医家对于本病的认识略有差异。从不同中医外科流派的角度,比较和总结全国不同流派治疗糖尿病足溃疡的临证经验、特色技术及学术思想,探讨其病因病机、辨证思路与特色疗法。参考文献42篇。Diabetic foot ulcer(DFU) is the most common and serious complication of diabetes mellitus,and is one of the main causes of disability and death in diabetic patients. Usually,it is difficult to treat and DFU lasts for a long time. Traditional Chinese medicine(TCM) has unique advantages in the treatment for DFU. Nowadays,according to most researchers,DFU pertains to the category of “digital gangrene” and“sinew gangrene” in TCM. It is a disease of root deficiency and branch excess,and dampness,heat,toxin and stasis are the major pathological factors,mainly involving liver,spleen and kidneys. During syndrome differentiation both the local and the whole should be aimed at,with the local differentiation as the major one. During the treatment,DFU should be treated according to different stages,and treated externally and internally. Modern TCM has new understanding of DFU,but the understandings by different schools and doctors are slightly different from each other. From the perspective of differentschools of TCM surgery,the clinical experience,special technique and academic thought of treating DFU by different schools in China were compared and summarized,to discusses the disease cause,disease mechanism,syndrome differentiation thought and special therapy of DFU. Forty-two papers are cited in the references.
