非静脉曲张性消化道出血患者经止血夹联合内镜下氩离子凝固术治疗的疗效及安全性分析

摘　　要：目的:分析止血夹联合内镜下氩离子凝固术治疗非静脉曲张性消化道出血患者的效果。方法:收集2019年7月~2021年1月非静脉曲张性消化道出血者90例,接受止血夹联合内镜下氩离子凝固术治疗的45例为研究组,以1:1比例匹配接受内镜下氩离子凝固术治疗的45例为对照组,比较两组患者及时止血率、再出血率、止血有效率、并发症状况、住院时间、呕血消失时间、肠鸣音复常时间、大便潜血转阴时间及术前、术后1 d凝血酶原时间(Prothrombin time,PT)、纤维蛋白原(Fibrinogen,FBG)、活化部分凝血酶原时间(Activated partial prothrombin time,APTT)、白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、C反应蛋白(C reactive protein,CRP)水平。结果:研究组及时止血率、止血有效率高于对照组(P<0.05);研究组住院时间、呕血消失时间、肠鸣音复常时间、大便潜血转阴时间短于对照组(P<0.05);术后1 d研究组FBG高于对照组,APTT、PT短于对照组(P<0.05);术后1 d研究组血清TNF-α、CRP、IL-6水平低于对照组(P<0.05);研究组并发症发生率少于对照组(P<0.05)。结论:止血夹联合内镜下氩离子凝固术治疗非静脉曲张性消化道出血者效果显著,可提高止血效果,缩短康复进程,改善凝血功能、炎性反应,降低并发症发生率。Objective:To analyze the effect of hemostatic clip combined with endoscopic argon ion coagulation in the treatment of non-varices gastrointestinal bleeding.Methods:A total of 90 patients with non-varices gastrointestinal hemorrhage from July 2019 to January 2021 were collected.45 patients who received hemostatic clamp combined with endoscopic argon ion coagulation were used as the research group.A 1:1 ratio of forty-five cases treated with argon ion coagulation were used as the control group.The in time to stop bleeding,rebleeding rate,effective rate of hemostasis,complication status,hospitalization time,hematemesis disappearance time,bowel sounds return to normal,stool occult blood to become negative time,preoperative and postoperative 1 d prothrombin time(PT),fibrinogen(FBG),activated partial prothrombin time(APTT),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C reactive protein(CRP)levels were compared between the two groups.Results:The study group's timely hemostasis rate and effective rate of hemostasis were higher than those of the control group(P<0.05);the study group's hospitalization time,hematemesis disappearance time,bowel sounds normalized time,and stool occult blood conversion time were shorter than those of the control group(P<0.05)1 day after operation,FBG in the study group was higher than that in the control group,APTT and PT were shorter than those in the control group(P<0.05);1 day after operation,the serum TNF-α,CRP and IL-6 levels in the study group were lower than those in the control group(P<0.05);the incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusion:The hemostatic clip combined with endoscopic argon ion coagulation is effective in treating patients with non-varices gastrointestinal hemorrhage.It can improve the hemostasis effect,shorten the rehabilitation process,improve the coagulation function,inflammatory reaction,and reduce the incidence of complications.
