睡眠结构紊乱与前、后循环脑梗死的相关性研究

摘　　要：目的通过比较前、后循环脑梗死患者睡眠结构中各期所占比例,探讨前、后循环脑梗死患者睡眠结构的不同变化。方法将48例首发急性脑梗死(ACI)患者按头颅磁共振(MRI)检查结果分为A组(前循环脑梗死组)和B组(后循环脑梗死组)。两组均经整夜睡眠呼吸监测后,将脑电图(EEG)结果进行分期分析。将两组患者各期所占百分率进行比较。结果两组患者W期、R期、N3期所占百分率差异有统计学意义(P<0.05),N1+N2期所占百分率差异无统计学意义(P>0.05)。结论前、后循环脑梗死患者均存在不同程度的睡眠结构紊乱。后循环梗死组W期、R期百分率均大于前循环梗死组,但N3期小于前循环梗死组,两组N1+N2期所占百分率差异无统计学意义。Objective To explore the different changes of sleep structure disorder in patients with anterior and posterior circulation cerebral infarction by comparing the proportion of different stages in sleep structure.Method According to the results of cranial MRI,forty-eight patients with first acute cerebral infarction(ACI)were divided into group A(anterior circulation cerebral infarction group)and group B(posterior circulation cerebral infarction group).The electroencephalogram EEG results of both groups were analyzed stage by stage after overnight sleep respiratory monitoring.The percentage of each stage was compared between the two groups.Results The differences in the percentages of stage W,stage R and stage N3 between the two groups were statistically significant(P<0.05);there was no significant difference in the percentages of stage N1+N2(P>0.05).Conclusion The patients with cerebral infarction of anterior and posterior circulation have different degree of sleep disorder.In the posterior circulation infarction group,the percentages in the W and R phases were higher than those in the anterior circulation infarction group,but in the N3 phase were lower than those in the anterior circulation infarction group.There was no statistical difference in the percentages in the N1+N2 phases between the two groups.
