摘 要:目的 探讨急性期脑小血管病(CSVD)患者进行双任务(DT)步行时,不同类型认知任务下认知-运动干扰(CMI)发生的情况。方法按照影像学表现共选取急性发病、肌力正常的CSVD患者12例,性别、年龄、受教育程度匹配的正常对照组12例,进行自然步态下直线行走18m距离,即单任务(ST)行走,以及步行同时分别执行2个难度的连续减法任务(SS)、单词生成任务(WLG)。采用超高分辨率3D结构光传感器搭配前沿AI深度学习算法的运动功能定量评价系统收集单、双任务步行的26个步态参数并进行比较。结果①组内比较,与ST比较,正常组执行DT步态参数中分别有14、13、15个指标有差异(P<0.05);CSVD组分别有2、1、0个指标有差异(P<0.05)。②组间比较,与正常组比较,CSVD组ST时13个步态指标有差异(P<0.05),DT时,SS有步态参数变化和计算数量的减少,无差异,WLG说动物名数量有差异(P<0.05)。结论执行ST时,CSVD组步态参数较正常组明显异常;执行DT时,正常组为认知干扰步态模式,CSVD组为认知运动相互干扰模式;WLG较SS任务简单,不同难度SS未显示出差别。Objective To investigate the occurrence of cognitive-motor interference(CMI) under different types of cognitive tasks in patients with acute cerebral small vessel disease(CSVD) during dual task(DT) walking.Methods A total of 12 CSVD patients with acute onset and normal muscle strength and 12 normal controls matched with gender,age and education level were selected according to imaging findings as subjects.The subjects were asked to walk a distance of 18 m in a straight line in a natural gait,that is,single task(ST) walking,and to perform two successive subtraction tasks(serial subtraction,SS) and word list tasks(word list) with two levels of difficulty while walking.generation,WLG.Twenty-six gait parameters for single-task and dual-task walking were collected and compared.The gait data collection tool used is a quantitative evaluation system of motor function with ultra-high resolution 3 D structured light sensor and cutting-edge AI deep learning algorithm.Results ① Comparison within each group:Compared with ST,there were differences in 14,13,and 15 indexes of gait parameters during DT in the normal group(P<0.05);There were differences in 2,1,and 0 indexes in the CSVD group(P<0.05).② Comparison between groups:Compared with the normal group,there were differences in 13 gait indicators in the CSVD group during ST(P<0.05).During DT,there were changes in gait parameters and a decrease in the number of calculations under the SS task,but there was no difference;In the WLG task,there is a difference in the number of animal names(P<0.05).Conclusion In the ST condition,the gait parameters of the CSVD group were significantly abnormal compared with the normal group;In the DT condition,the normal group was in the cognitive interference gait mode,and the CSVD group was in the cognitive motor mutual interference mode.The WLG task was simpler than the SS task,and the SS task of different difficulty no difference was shown.
