摘  要:目的探究多模态超声联合磁共振技术在早期乳腺癌鉴别中的应用价值。方法选择2018年1月至2020年12月于昆山市中医医院接受诊断的80例疑似乳腺癌患者为研究对象,分别对其开展多模态超声(包括应变式弹性成像和剪切波弹性成像检查)和磁共振检查,以病理学检测结果为金标准,分析良、恶性乳腺癌病变患者超声及磁共振检测指标差异,探讨多模态超声、磁共振、多模态超声联合磁共振3种检测方式在乳腺癌良恶性鉴别中的临床价值。结果恶性组患者应变式弹性成像(SE)参数弹性评分和应变率比值(SR)均高于良性组患者(P<0.05);恶性组患者剪切波弹性成像(SWE)参数的弹性模量平均值(Emean)、弹性模量标准差(SD)及弹性模量与周围正常乳腺组织的比值(Ratio)均高于良性组患者(P<0.05);恶性组患者MRI参数中微血管密度(MVD)和强化率明显高于良性组患者,峰值时间低于良性组(P<0.05);分别绘制SE、SWE、SE+SWE、MRI、SE+SWE+MRI对早期乳腺癌的诊断ROC曲线,计算其AUC分别为0.6250、0.6063、0.6938、0.6438、0.7625,SE+SWE+MRI对早期乳腺癌的诊断敏感度明显高于其他检测手段(P<0.05)。结论多模态超声以及磁共振对早期乳腺癌均具有较好的鉴别诊断价值,但如果能将两种方式联合,则能够明显提高诊断的灵敏度,对提高乳腺癌的早期筛查率具有积极意义。Objective To explore the application value of multimodal ultrasound combined with magnetic resonance technology in the identification of early breast cancer.Methods Patients with suspected breast cancer who were diagnosed in from January 2018 to December 2020 ultimodal ultrasound(strain elastography and shear wave elastography examination)and magnetic resonance examination athological examination results were used as the gold standardto analyze the clinical value of the three detection methods multimodal ultrasound,magnetic resonance,multimodal ultrasound+magnetic resonance in the differentiation of breast cancer.Results Strain lastography(SE)parameter were significantly higher than those in the benign group(P<0.05).The shear wave elastography(SWE)parameters Emean,SD and Ratio values of the patients in the malignant group were significantly higher than those in the benign group(P<0.05).The MVD and enhancement rates MRI parameters the malignant group were significantly higher than those in the benign group,and the peak time was significantly lower than that in the benign group(P<0.05)The ROC curve of SE+SWE,MRI,SE+SWE+MRI for the diagnosis of early breast cancer 0.6250,0.6063,0.6938,0.6438,0.7625,SE+SWE+MRI ha sensitivity for the diagnosis of early breast cancer than other detection methods(P<0.05).Conclusions Both multimodal ultrasound and magnetic resonance have good differential diagnosis value for early breast cancerhe sensitivity of diagnosis can be significantly improvedthe early of breast cancer.
