鼻咽血管纤维瘤中GLUT-1与PCNA表达及其起源

摘　　要：目的探究鼻咽血管纤维瘤(JNA)起源的本质。方法收集2006年7月—2017年12月诊治的资料完整的JNA患者20例、婴幼儿头面部血管瘤患者9例、头面部血管畸形患者20例、鼻息肉患者15例。采用免疫组织化学染色(IHC)检测上述患者标本的血管内皮细胞的葡萄糖转运蛋白-1(GLUT-1)和增殖细胞核抗原(PCNA)表达,并对4组数据进行统计学分析。结果GLUT-1和PCNA在JNA血管内皮细胞的表达阳性率为5.0%(1/20)、10.0%(2/20),低于在血管瘤组织中的阳性率[88.9%(8/9),55.6%(5/9)],差别均有统计学意义(P<0.05),而与组织血管畸形和鼻息肉的阳性率[0%(0/20)、5.0%(1/20);6.7%(1/15)、0%(0/15)]差别均无统计学意义(P>0.05)。结论JNA的本质可能是胚胎残余的血管畸形,而不是真性血管瘤。GLUT-1和PCNA在JNA的表达与血管瘤明显不同,与血管畸形和鼻息肉无明显差别,可作为鉴别血管畸形和血管瘤的标志。Objective To investigate the nature of juvenile nasopharyngeal angiofibroma(JNA).Methods From July 2006 to December 2017,20 cases of JNA,9 cases of head and neck hemangioma,20 cases of head and face vascular malformation and 15 cases with nasal polyp were admitted.The expression of glucose transporter-1(GLUT-1)and proliferating cell nuclear antigen(PCNA)in vascular endothelial cells of the above cases were detected by immunohistochemistry,and the data of the four groups were statistically analyzed.Results The positive rates of GLUT-1 and PCNA expression in the endothelial cells of JNA were 5%(1/20),10%(2/20),lower than those in hemangioma which were 88.9%(8/9),55.6%(5/9).It was a significant difference(P<0.05),but there was no significant difference between them and those in vascular malformation they were 0%(0/20),5.0%(1/20),and were 6.7%(1/15)and 0%(0/15)in nasal polyp,the difference between JNA and vascular malformation(P>0.05)was not significant.Conclusion The essence of JNA may be the residual vascular malformation of the embryo rather than hemangioma.The expression of GLUT-1 and PCNA in JNA was significantly different from that of hemangioma,and had no significant difference from that of vascular malformation and nasal polyp.It can be used as a marker to differentiate vascular malformation and hemangioma.
