福建省莆田市11 924例女性人乳头瘤病毒感染的基因亚型分析

摘　　要：目的分析福建省莆田市女性人乳头瘤病毒(HPV)的感染状况、基因亚型及年龄分布特征,为宫颈癌的早筛和临床诊疗提供参考。方法收集2021年1月—2022年1月莆田学院附属医院11924例门诊、住院和体检女性的宫颈脱落细胞标本,应用聚合酶链式反应(PCR)和流式荧光杂交法检测样本HPV 27种基因亚型,分析不同年龄段HPV的感染状况、高危型和低危型分布情况、单一感染、双重感染和多重感染的情况。结果11924份标本中,2667份检查出感染HPV,感染率达22.37%。其中≤20岁患者的为51.95%,61~70岁患者的为29.38%。不同年龄段HPV感染率比较,差别有统计学意义(χ^(2)=60.85,P<0.01)。2667例感染者中,单一感染1993例,双重感染501例。此外,检出HPV感染基因3580次,其中高危型检出频数为2643次(73.81%),居前三位的高危型依次为HPV-52、HPV-53和HPV-58。结论福建省莆田市就诊女性HPV感染率达22.37%,且每个年龄段高危型的检出率均高于低危型。卫生部门和医疗机构应推广多价HPV疫苗,加强青年和中老年女性的HPV定期筛查和科普宣传,对于宫颈癌预防和早诊具有重要作用。Objective To analyze the infection status,gene subtypes and age distribution of human papilloma virus(HPV)among women in Putian City,Fujian Province for providing reference for early screening and clinical diagnosis and treatment of cervical cancer.Methods Cervical exfoliated cell samples from 11924 outpatients,inpatients and physical examination women in the Affiliated Hospital of Putian University from January 2021 to January 2022 were collected.27 genotypes of HPV in the samples were detected by polymerase chain reaction(PCR)and flow fluorescence hybridization.The infection status,distribution of high-risk and low-risk types,single infection,double infection and multiple infection of HPV in different age groups were analyzed.Results Of the 11924 samples,2667 were infected with HPV,with an infection rate of 22.37%:among them,the infection rate of≤20 years old was 51.95%and the infection rate of 61-70 years old was 29.38%.There was significant differences in HPV infection rate among different age groups(χ^(2)=60.85,P<0.01).Among the 2667 cases,there were 1993 cases of single infections and 501 cases of double infections.In addition,a total of 3580 HPV infection genes were detected,of which 2643 high-risk types(73.81%)were detected.The top three high-risk types were HPV-52,HPV-53 and HPV-58.Conclusion The infection rate of HPV among female patients in this area reached 22.37%,and the detection rate of high-risk types was higher than that of low-risk types in each age group.Health departments and medical institutions should promote multivalent HPV vaccine and strengthen regular HPV screening and science popularization among young and middle-aged women,which plays an important role in cervical cancer prevention and early diagnosis.
