摘 要:目的探讨应用3.0T磁共振R_(2)^(*)与T_(1)-mapping技术对长期反复输血患者的肝脏铁过载情况进行定量的分析,量化评估进行去铁治疗前、后及过程中的肝脏铁沉积的程度。方法对30例因长期输血造成肝脏铁过载的患者,在治疗前后进行磁共振检查,测得R_(2)^(*)值及T_(1)值,分析治疗前后R_(2)^(*)值及T_(1)-mapping与血清铁蛋白浓度及转铁蛋白饱和度之间的对应关系,评估去铁治疗的效果。结果30例铁过载患者肝脏R_(2)^(*)值与SF相关系数r 2为0.678,差异有统计学意义(P<0.05);肝脏T_(1)值与SF相关系数r为-0.625,差异有统计学意义(P<0.05);去铁治疗前、后的肝脏R_(2)^(*)值、T_(1)值差异均有统计学意义。结论联合R_(2)^(*)与T_(1)-mapping对肝脏铁过载的检测准确性更高,与血清铁蛋白有高度一致性,并且可以有效的评估去铁治疗前、后肝脏铁过载的程度。Objective In this study,3.0 T Magnetic resonance R_(2)^(*)and T_(1)-Mapping were used to quantitatively analyze the liver iron overload of patients with long-term repeated blood transfusion,and to quantitatively evaluate the degree of liver iron deposition before,after and during the treatment of iron removal.Methods 30 patients with liver iron overload caused by long-term blood transfusion were examined by magnetic resonance imaging before and after treatment,R_(2)^(*)and T_(1) were measured,and the correlation between R_(2)^(*)and T_(1)-mapping,serum ferritin concentration and transferrin saturation before and after treatment was explored to evaluate the effect of iron removal treatment.Results In 30 patients with iron overload,the correlation coefficient r 2 between liver R_(2)^(*)and SF was 0.678,with statistical significance(P<0.05).The correlation coefficient r between T_(1) value of liver and SF was-0.625,with statistically significant(P<0.05).There were significant differences in R_(2)^(*)and T_(1) values of liver before and after iron removal.Conclusion The combination of R_(2)^(*)and T_(1)-mapping has a higher accuracy in detecting iron overload in liver,highly consistent with serum ferritin,which can effectively evaluate the degree of iron overload in liver before and after iron removal treatment.
