摘　　要：目的:探讨血清微管相关蛋白1轻链3-Ⅱ(LC3-Ⅱ)、半胱氨酸天冬氨酸特异性蛋白酶-3(Caspase-3)及高迁移率族蛋白B1(HMGB1)在不同程度急性胰腺炎(AP)中的表达及意义。方法:选取AP患者135为病例组,根据病情严重程度又分为轻症急性胰腺炎(MAP)组、中度重症急性胰腺炎(MSAP)组以及重症急性胰腺炎(SAP)组三个亚组,每组45例。选取同期体检健康者135例为对照组。采用酶联免疫吸附法(ELISA)对血清LC3-Ⅱ、Caspase-3、HMGB1表达水平进行测定。采用急性生理学和慢性健康状况评估Ⅱ(APACHEⅡ)对病例组患者进行评估。比较病例组与对照组血清LC3-Ⅱ、Caspase-3、HMGB1表达水平。比较不同程度AP患者血清LC3-Ⅱ、Caspase-3、HMGB1表达水平及APACHEⅡ评分。采用Spearman法分析血清LC3-Ⅱ、Caspase-3、HMGB1与AP患者APACHEⅡ评分的相关性。采用受试者工作特征(ROC)曲线分析血清LC3-Ⅱ、Caspase-3、HMGB1对AP的预测价值。结果:与对照组相比,病例组患者血清LC3-Ⅱ、Caspase-3、HMGB1表达水平明显升高(均P<0.05)。血清LC3-Ⅱ、Caspase-3、HMGB1表达水平及APACHEⅡ评分在不同程度AP患者中由高到低依次为SAP组、MSAP组、MAP组,各组间比较差异有统计学意义(均P<0.05)。血清LC3-Ⅱ、Caspase-3、HMGB1与AP患者APACHEⅡ评分呈正相关(r=0.704、0.642、0.723,均P<0.05)。血清LC3-Ⅱ、Caspase-3、HMGB1对AP具有良好的预测价值。结论:血清LC3-Ⅱ、Caspase-3、HMGB1水平在AP患者中升高,且与病情严重程度呈正相关,对AP具有良好的预测价值。Objective:To investigate the expression and significance of serum microtubule associated protein 1 light chain 3-Ⅱ(LC3-Ⅱ),Caspase-3 and high mobility group protein B1(HMGB1)in different degrees of acute pancreatitis(AP).Methods:A total of 135 patients with AP were selected as case group and divided into three subgroups according to the severity of AP:mild acute pancreatitis(MAP)group,moderate severe acute pancreatitis(MSAP)group and severe acute pancreatitis(SAP)group,with 45 cases in each group.A total of 135 healthy subjects during the same period were selected as control group.Serum levels of LC3-Ⅱ,Caspase-3 and HMGB1 were measured by ELISA.Patients in the case group were evaluated by using APACHEⅡ.Serum levels of LC3-Ⅱ,Caspase-3 and HMGB1 were compared between the case group and the control group.Serum levels of LC3-Ⅱ,Caspase-3,HMGB1 and APACHEⅡscores were compared in patients with different degrees of AP.The correlation between serum LC3-Ⅱ,Caspase-3,HMGB1 and APACHEⅡscore in AP patients was analyzed by Spearman’s method.ROC curve was used to analyze the predictive value of serum LC3-Ⅱ,Caspase-3 and HMGB1 for AP.Results:Compared with the control group,the levels of serum LC3-Ⅱ,Caspase-3 and HMGB1 in the case group were significantly increased(all P<0.05).The levels of serum LC3-Ⅱ,Caspase-3,HMGB1 and APACHEⅡscore in different degrees of AP patients were in the order of SAP group,MSAP group and MAP group from high to low,and the differences between the groups were statistically significant(all P<0.05).There was a positive correlation between serum LC3-Ⅱ,Caspase-3,HMGB1 and APACHEⅡscore in AP patients(r=0.704,0.642,0.723,all P<0.05).Serum LC3-Ⅱ,Caspase-3 and HMGB1 had good predictive value for AP.Conclusion:The levels of serum LC3-Ⅱ,Caspase-3 and HMGB1 in AP patients are increased,and they are positively correlated with the severity of the disease,which has good predictive value for AP.
