摘 要:目的 探讨多项肿瘤标志物联合筛查在恶性肿瘤二级预防中的应用研究。方法 选取2019年1月至2021年6月间上海市崇明区第三人民医院进行恶性肿瘤疾病筛查的2 200例健康筛查人群,采用电化学发光法检测甲胎蛋白(AFP)、糖类抗原15-3(CA15-3)、CA125、CA19-9和癌胚抗原(CEA)水平阳性率、恶性肿瘤确诊情况,将肿瘤标志物阳性者纳入阳性组,阴性者纳入阴性组,比较两组各项血清肿瘤标志物水平,不同性别、年龄受试人群的肿瘤标志物结果对恶性肿瘤的诊断情况。结果 AFP、CA15-3、CA125、CA19-9和CEA阳性检出率分别为10.7%(235例)、5.4%(118例)、1.2%(26例)、7.2%(158例)和5.0%(109例)。共70例患者出现两种以上肿瘤标志物检测阳性,阳性率为3.2%,其中12例(0.5%)经病理学、影像学等检查确诊为早期或中期肿瘤,恶性肿瘤8例(0.4%)。肿瘤标志物阳性组的AFP、CA15-3、CA125、CA19-9和CEA水平均高于阴性组,差异均有统计学意义(均P<0.05)。男性群体CA19-9阳性率低于女性,CEA阳性率高于女性,差异均有统计学意义(均P<0.05)。51~60岁人群CA15-3、CA19-9和CEA阳性率均高于40~50岁,差异均有统计学意义(均P<0.05)。结论 健康体检人群二级预防中,肿瘤标志物AFP、CA15-3、CA125、CA19-9和CEA均为高表达,其中AFP阳性率表达最高,不同性别、年龄人群的多项肿瘤标志物水平有差异性,利于临床中二级预防,早发现早治疗,弱化恶性肿瘤疾病继续恶化的风险。Objective To explore the application of combination screening of multiple tumor markers in the secondary prevention of malignant tumors. Methods A total of 2200 individuals who underwent health screening for malignant tumor were selected at Shanghai Chongming District Third People’s Hospital from January 2019 to June 2021. The positive rate of serum tumor marker of alpha-fetoprotein(AFP), carbohydrate antigen 15-3(CA15-3), carbohydrate antigen 125(CA125), carbohydrate antigen 19-9(CA19-9) and carcinoembryonic antigen(CEA) were detected using electrochemiluminescence method and the diagnosis of malignant tumors were recorded. According to the test results, participants with positive tumor markers detection were included in a positive group, and patients with negative tumor markers detection were included in a negative group. The serum levels of tumor markers, and the detection efficacy of tumor markers in participants of different gender and age for malignant were compared two groups. Results The positive rates of AFP, CA15-3, CA125, CA19-9 and CEA were 10.7%(235 cases), 5.4%(118 cases), 1.2%(26 cases), 7.2%(158 cases), and 5.0%(109 cases), respectively. A total of 70 patients were detected positive for more than two tumor markers, with a total positive rate of 3.2%. Among them, 12 patients(0.5%) were confirmed as early or intermediate tumors by pathological and imaging examinations, and 8 patients(0.4%) had malignant tumors. The levels of AFP, CA15-3, C125, CA19-9 and CEA were higher in tumor marker positive group than those in the negative group(all P<0.05). The positive rates of CA125 and CA19-9 were lower in males than in females, while the positive rates of CEA were higher in males than in females(all P<0.05). The positive rates of CA15-3, CA19-9 and CEA were higher in patients aged 51 to 60 years than in patients aged 40 to 50 years(all P<0.05).
