摘 要:目的探讨普瑞巴林对大鼠急、慢性臂丛神经痛模型的作用及机制。方法雄性SD大鼠36只,月龄2~3个月,体质量250~300 g,采用随机数字表法将其随机分为3组:模型组、急性治疗组(POD1组)和慢性治疗组(POD12组),每组各12只。臂丛神经下干内注射蛇毒建立臂丛神经损伤模型,POD1组于造模成功后第1日起口服普瑞巴林30 mg/kg,连续给药6 d。POD12组于术后第12日起口服普瑞巴林30 mg/kg,连续给药6 d。各组于术前和术后3、6、9、12、15、20、30和60 d观察大鼠左前肢及右后肢对机械刺激的疼痛反应阈值,同时录像观察自发痛行为学表现。术后14、60 d每组各取2只大鼠行主动脉灌注后取材,电镜下观察臂丛神经和颈髓(C_(8)~T_(1))的超微结构改变。结果与模型组比较,POD1组术后6、9、12、15、20、30及60 d左前肢机械痛阈提高,POD12组术后30、60 d左前肢机械痛阈提高,差异均有统计学意义(P均<0.05)。与模型组比较,POD1组术后20、30及60 d右后肢机械痛阈提高,POD12组术后30、60 d右后肢机械痛阈提高,差异均有统计学意义(P均<0.05)。与模型组比较,POD1组术后6、9、12、15、20、30及60 d探究行为次数和时间增加,修饰行为次数和时间减少;POD12组术后30及60 d探究行为次数和时间增加,修饰行为次数和时间减少,差异有统计学意义(P均<0.05)。电镜下观察各组术后14、60 d臂丛神经及颈髓(C_(8)~T_(1))有明显脱髓鞘改变。POD1组和POD12组术后60 d电镜下臂丛神经下干脱髓鞘好转,但(C_(8)~T_(1))有髓纤维改善不明显。结论普瑞巴林可通过改善臂丛神经脱髓鞘对大鼠急、慢性臂丛神经痛有治疗作用,且对急性臂丛神经痛效果更明显。Objective To investigate the therapeutic effect and mechanism of pregabalin on acute and chronic brachial plexus neuralgia in a rat model.Methods Thirty-six adult male Sprague-Dawley rats,aged 2-3 months,weight 250-300 g,were randomly divided into 3 groups using random number table method:control group,acute treatment group(group POD1)and chronic treatment group(group POD12),12 cases in each group.Cobra venom was injected into the lower trunk of brachial plexus to establish the rat model of brachial plexus neuralgia.Pregabalin was administered intragastrically,30 mg/kg in group POD1 from the 1st day after establishing the model and the 12th day after establishing the model respectively,once a day for 6 consecutive days.Mechanical withdrawal threshold(MWT)was tested with von Frey filaments in the right hindpaw and the left forepaw.Video recording was used to analyze the spontaneous behaviors before the operation and 3,6,9,12,15,20,30 and 60 days after the operation.Two rats were taken from each group for electron microscope examination at 14 and 60 days after the operation.Ultrastructure changes of the brachial plexus and cervical cord(C_(8)-T_(1))were observed after perfused intracardially,by electron microscope.Results Compared with the control group,MWT of the left forelimb was significantly increased at 6,9,12,15,20,30 and 60 days after the operation in group POD1,and it was increased at 30 and 60 days after the operation in group POD12(all P<0.05).Compared with the control group,MWT of the right hindpaw was significantly increased at the 20,30 and 60 days after the operation in group POD1,and it was increased at 30 and 60 days after the operation in group POD12(all P<0.05).Compared with the control group,the number and time of exploratory activity were significantly increased,but the number and time of grooming behavior were significantly decreased at 6,9,12,15,20,30 and 60 days after the operation in group POD1;while the number and time of exploratory activity were significantly increased,but the number and
