摘 要:目的 探讨胰岛素样生长因子Ⅰ(IGF-Ⅰ)和胸苷激酶1(TK-1)与脑胶质瘤的相关性。方法 选取2012年8月-2020年8月于都江堰市人民医院行手术治疗的脑胶质瘤患者120例作为研究对象,根据胶质瘤级别分为低级组(n=53)、高级组(n=67),另选取行颅脑手术的良性脑膜瘤患者39例作为对照组,比较各组手术前后血清IGF-Ⅰ、TK-1水平变化情况。计算血清IGF-Ⅰ、TK-1诊断脑胶质瘤的敏感度和特异度,Pearson法分析血清IGF-Ⅰ、TK-1水平与脑胶质瘤等级的相关性。结果 手术前,胶质瘤组血清IGF-Ⅰ、TK-1水平均显著高于对照组(P<0.05)。且术前高级组脑胶质瘤患者血清IGF-Ⅰ、TK-1均显著高于低级组。IGF-Ⅰ、TK-1联合检测时灵敏度最高、约登指数最大。血清IGF-Ⅰ及TK-1表达水平与脑胶质瘤级别呈正相关(r=0.725,0.663,P<0.05)。结论 脑胶质瘤患者血清IGF-Ⅰ、TK-1均显著高于脑膜瘤患者,二者与胶质瘤分级具有较好相关性,行颅脑手术后二者水平均出现降低,术前血清IGF-Ⅰ、TK-1两者联合可以准确预测脑胶质瘤分级。Objective To explore the correlation between insulin-like growth factorⅠ(IGF-Ⅰ),thymidine kinase 1(TK-1)and glioma.Methods A total of 120 glioma patients undergoing surgical treatment in the hospital from August 2012 to August 2020 were enrolled as the research objects.According to the grading of glioma,they were divided into low-grade group(n=53)and high-grade group(n=67).Another 39 patients with benign meningiomas undergoing craniocerebral surgery were enrolled as control group.The changes in levels of serum IGF-Ⅰand TK-1 before and after surgery were compared among all groups.The sensitivity and specificity of serum IGF-Ⅰand TK-1 in the diagnosis of glioma were calculated The correlation between levels of glioma grading and serum IGF-Ⅰ and TK-1 levels was analyzed by Pearson method.Results Before surgery,levels of serum IGF-Ⅰand TK-1 in glioma group were significantly higher than those in control group(P<0.05).Before surgery,levels of serum IGF-Ⅰand TK-1 in high-grade group were significantly higher than those in low-grade group(P<0.05).The sensitivity and Yoden index were the highest when IGF-Ⅰand TK-1 were combined Serum IGF-Ⅰand TK-1 expression levels are positively correlated with the level of glioma(r=0.725,0.663,P<0.05).Conclusion The levels of serum IGF-Ⅰand TK-1 in glioma patients are significantly higher than those in meningioma patients.Both of them have a good correlation with the grade of glioma,After craniocerebral surgery,their levels are decreased.The combination of serum IGF-Ⅰand TK-1 before surgery can more accurately predict glioma grading.
