肾上腺区嗜铬细胞瘤与肾上腺皮质腺瘤CT征象临床对比

摘　　要：目的 分析并对比肾上腺区嗜铬细胞瘤与肾上腺皮质腺瘤CT征象,明确CT的诊断价值,提升诊断准确性。方法 回顾性分析2018年2月至2020年10月期间收治的80例经病理及临床实验室检查确诊的肾上腺肿瘤患者临床相关资料,统计并对比肾上腺区嗜铬细胞瘤组与肾上腺皮质腺瘤组患者基线资料及相关检查结果。结果 80例肾上腺肿瘤患者中肾上腺嗜铬细胞瘤54例,肾上腺皮质腺瘤26例,其中醛固醇腺瘤19例,无功能腺瘤4例,库欣腺瘤3例;肾上腺区嗜铬细胞瘤组患者体质量指数较肾上腺皮质腺瘤组小,差异有统计学意义(P<0.05);肾上腺区嗜铬细胞瘤组肿瘤长径、宽径均较肾上腺皮质腺瘤组长,差异有统计学意义(P<0.05);肾上腺区嗜铬细胞瘤组平扫CT值、动脉期CT值及静脉期CT值均较肾上腺皮质腺瘤组高,差异有统计学意义(P<0.05);逐个将自变量纳入行二元Logistic回归分析,后将全部自变量纳入建立多元回归模型行多元回归分析,结果显示,宽径、平扫CT值、动脉期CT值对肾上腺区嗜铬细胞瘤鉴别有影响(OR>1,P<0.05);作ROC曲线分析,结果显示,宽径、平扫CT值、动脉期CT值预测肾上腺区嗜铬细胞瘤组与肾上腺皮质腺瘤的AUC>0.90,预测价值较理想。结论 肾上腺区嗜铬细胞瘤与肾上腺皮质腺瘤的CT征象存在一定差异性,CT平扫及增强扫描在肾上腺区肿瘤的鉴别诊断中具有重要意义。Objective to analyze and compare the CT features of adrenal pheochromocytoma and adrenocortical adenoma.Methods the clinical data of 80patients with adrenal tumors diagnosed by pathology and clinical laboratory examination from February 2018to October 2020 were retrospectively analyzed,and the data of patients with adrenal pheochromocytoma group and adrenal cortical adenoma group were statistically compared.Results Among the 80patients with adrenal tumors,there were 54cases of adrenal pheochromocytoma and 26cases of adrenocortical adenoma,including 19cases of aldosterone adenoma,4cases of nonfunctional adenoma,and 3cases of Cushing’s adenoma;the body mass index of patients in the adrenal pheochromocytoma group was smaller than that in the adrenocortical adenoma group,and the difference was statistically significant(P<0.05);the length and width of tumors in the adrenal pheochromocytoma group were longer than those in the adrenocortical adenoma group,and the difference was statistically significant(P<0.05).The difference was statistically significant(P<0.05);the pheochromocytoma group in the adrenal area had higher CT values in the scan,arterial CT and venous CT values than the adrenal cortical adenoma group,and the difference was statistically significant(P<0.05);The independent variables were included in the binary logistic regression analysis one by one,and then all independent variables were included in the establishment of multiple regression model for multiple regression analysis.The results showed that the wide diameter,plain CT value and arterial CT value had an impact on the differentiation of adrenal pheochromocytoma(or>1,P<0.05);ROC curve analysis showed that the AUC of pheochromocytoma group and adrenocortical adenoma in adrenal region predicted by wide diameter,plain CT value and arterial CT value was more than 0.90.Conclusion the CT signs of adrenal pheochromocytoma and adrenal cortical adenoma are different.Plain and enhanced CT scanning are of great significance in the differential diagnosis of ad
