摘　　要：目的:探讨红细胞分布宽度(RDW)、同型半胱氨酸(Hcy)在胃癌诊断中的临床意义。方法:回顾性分析62例胃癌患者和62例健康对照者的临床资料,比较两组RDW及Hcy的差异,分析胃癌患者RDW、Hcy表达水平与病理参数的关系,建立二元多因素Logistic回归模型筛选胃癌的危险因素,同时绘制ROC曲线分析RDW、Hcy单独及联合检测对胃癌的诊断价值。结果:胃癌患者RDW和Hcy的表达水平高于健康对照组(P<0.05)。胃癌患者RDW和Hcy的表达水平与病理分期(pTNM)、淋巴结转移、远处转移有关(P<0.05)。二元多因素Logistic回归分析RDW(OR=1.685,95%CI:1.014~2.800)、Hcy(OR=2.146,95%CI:1.575~2.923)为胃癌的独立危险因素(P<0.05)。RDW、Hcy单独及联合诊断胃癌的ROC曲线下面积分别为0.686、0.749、0.839。灵敏度分别为41.9%、64.5%、74.2%;特异度86.8%、83.9%、90.3%。结论:RDW、Hcy两项联合检测可以有效提高胃癌患者诊断效能,具有良好的辅助诊断价值。Objective:To investigate the implications of erythrocyte distribution width(RDW) and homocysteine(Hcy) level in gastric cancer.Methods:Retrospective analysis was performed in cases of 62 gastric cancer and 62 healthy controls.The difference in RDW and Hcy was compared between the two groups, and the relationship of expression levels of RDW and Hcy with pathological parameters was analyzed in gastric cancer patients.Then a binary multivariate logistic regression model was established to screen the risk factors for gastric cancer, and ROC curve was drawn to analyze the diagnostic value of single RDW and Hcy indicator or combined use of the two indicators in diagnosis of gastric cancer.Results:The expression levels of RDW and Hcy were higherin gastric cancer patients than in healthy controls(P<0.05).RDW and Hcy expression was associated with pathological stage(pTNM),lymph node metastasis and distant metastasis in gastric cancer patients(P<0.05).Binary multivariate logistic regression analysis showed that RDW and Hcy were independent risk factors for gastric cancer(OR=1.685,95%CI:1.014-2.800 for RDW;OR=2.146,95%CI:1.575-2.923 for Hcy, both P<0.05).The area under ROC curve for diagnosis of gastric cancer by single RDW and Hcy combined use of the two indicators was 0.686,0.749 and 0.839,respectively.The sensitivity and specificity was 41.9%,64.5% and 74.2%,and 86.8%,83.9% and 90.3%,respectively.Conclusion:Combined detection of RDW and Hcy can effectively improve the diagnostic efficiency of, and be auxiliary diagnostic value for gastric cancer.
