摘  要:目的 探究炎症性肠病(IBD)患者肠黏膜组织中miR-129与程序性细胞死亡因子4(PDCD4)的表达关系及临床意义。方法 选择2017年7月至2020年1月万宁市人民医院收治的176例活动期IBD患者设为IBD组,其中94例为溃疡性结肠炎(UC)患者(设为UC组),82例为克罗恩病(CD)患者(设为CD组);另选择同期在该院就诊的100例结肠息肉患者设为对照组。在IBD组患者行肠镜检查时取肠黏膜病变组织,在对照组患者行肠镜下息肉切除术时取距息肉>5 cm的回肠或结肠黏膜活体组织。采用实时荧光定量PCR法检测肠黏膜组织中miR-129和PDCD4的表达水平,并分析两者的相关性,以及其与患者疾病活动度的关系。采用ROC曲线评估肠黏膜组织中mi R-129和PDCD4诊断IBD的效能。结果 IBD组肠黏膜组织中miR-129、PDCD4的相对表达量分别为0.82±0.23、0.67±0.19,均低于对照组(1.36±0.35、1.02±0.27),2组的差异均有统计学意义(P均<0.05)。轻度UC组、中度UC组、重度UC组的miR-129和PDCD4表达水平依次降低,轻度CD组、中度CD组、重度CD组的miR-129和PDCD4表达水平也依次降低,差异均有统计学意义(P均<0.05)。UC组的miR-129和PDCD4表达水平均与改良Mayo评分呈负相关(r=-0.580、r=-0.595,P均=0.000),CD组的miR-129和PDCD4表达水平均与CD活动指数呈负相关(r=-0.508、r=-0.498,P均=0.000)。IBD组患者肠黏膜组织中miR-129与PDCD4的表达水平呈正相关(r=0.735,P=0.000)。IBD组患者肠黏膜组织中mi R-129和PDCD4诊断IBD的ROC曲线下面积(AUC)分别为0.759(95%CI:0.703~0.816)和0.867(95%CI:0.821~0.913),两者联合诊断的AUC为0.911(95%CI:0.874~0.947)。结论 IBD患者肠黏膜组织中miR-129和PDCD4的表达水平降低,且两者呈正相关。miR-129和PDCD4的表达水平与UC及CD患者的疾病活动度相关,有望作为诊断IBD的生物标志物。Objective This paper intends to study the expression relationship and significance of miR-129 and programmed cell death 4(PDCD4) in the intestinal mucosa of patients with inflammatory bowel disease(IBD).Methods A total of 176 patients with active IBD(assigned as the IBD group) who were admitted to the People’s Hospital of Wanning from July 2017 to January 2020 were selected,of which 94 were patients with ulcerative colitis(UC,assigned as the UC group) and 82 were patients with Crohn’s disease(CD,assigned as the CD group).Another 100 patients with colon polyps who were treated in this hospital in the same period were selected as the control group.The intestinal mucosal lesions tissue was obtained from the patients in the IBD group by intestinal endoscopic biopsy,and the ileum or colon tissue 5 cm away from the polyp was obtained from the patients in the control group during intestinal endoscopic polypectomy.The expression levels of mi R-129 and PDCD4 in intestinal mucosa were detected by real-time fluorescence quantitative PCR,and the correlation between the expression levels of mi R-29 and PDCD4 was analyzed,so was the relationship between the expression levels of mi R-29 and PDCD4 and disease activity of patients.The ROC curve was used to evaluate the efficacy of mi R-129 and PDCD4 in the diagnosis of IBD in the intestinal mucosal tissue.Results The relative expression of mi R-129 and PDCD4 in the IBD group are 0.82±0.23 and 0.67±0.19,respectively,lower than that in the control group(1.36±0.35 and 1.02±0.27),with a statistically significant difference(P<0.05).The expression levels of mi R-129 and PDCD4 in the mild UC group,the moderate UC group,and the severe UC group are decreased in turn,so are those of mi R-129 and PDCD4 in the mild CD group,the moderate CD group,and the severe CD group sequentially,with a statistically significant difference(P<0.05).The expression levels of mi R-129 and PDCD4 in the UC group are negatively correlated with the modified Mayo score(r=-0.580,r=-0.595,and P=0.000),while t
