摘 要:目的研究活髓切断术中盖髓剂的选择对儿童牙外伤致牙露髓的年轻恒牙疗效、牙龈健康状况及牙本质桥形成率的影响。方法选取遂宁市中心医院2019年1月-2021年1月收治的儿童牙外伤致牙冠折断露髓患者60例作为研究对象,按照随机分组原则分为观察组以及对照组,对照组采取氢氧化钙活髓切断术进行治疗,观察组采取生物陶瓷iRoot BP Plus活髓切断术进行治疗。比较两组患者的修复成功率、疼痛、牙本质桥形成率、牙齿变色率、牙龈健康状况之间的差异。结果观察组的1(χ^(2)=4.286,P=0.038)、3(χ^(2)=5.455,P=0.019)、6(χ^(2)=6.405,P=0.015)、12(χ^(2)=5.455,P=0.019)月的修复成功率高于对照组;观察组的1(t=4.292,P=0.001)、3(t=3.349,P=0.001)、6(t=8.876,P=0.001)月的视觉模拟评分(visual analogue scale,VAS)低于对照组;观察组的1(χ^(2)=4.286,P=0.038)、3(χ^(2)=5.554,P=0.018)、6(χ^(2)=5.079,P=0.024)、12(χ^(2)=6.667,P=0.010)月的牙本质桥形成率高于对照组;观察组的1(χ^(2)=4.286,P=0.028)、3(χ^(2)=4.043,P=0.044)、6(χ^(2)=5.455,P=0.020)、12(χ^(2)=5.963,P=0.015)月的牙齿变色率低于对照组;观察组的1(t=4.287,26.847,P<0.001)、3(t=4.899,9.859,P<0.001)、6(t=22.045,8.660,P<0.001)、12(t=15.191,20.176,P<0.001)月的牙龈指数(gingival index,GI)和龈沟出血指数(sulcus bleeding index,SBI)评分低于对照组。结论儿童牙外伤致牙冠折断露髓活髓切断术中使用生物陶瓷iRoot BP Plus进行盖髓,患者牙本质桥形成率较高,牙龈健康状况较好,治疗效果显著。Objective To study the effect of different pulp capping agent in pulpotomy treatment of young per-manent teeth with pulp exposure caused by dental trauma in children.Methods A total of 60 children with crown frac-ture and pulp exposure caused by dental trauma in our hospital from January 2019 to January 2021 were taken as the re-search object.These patients were randomly divided into observation group and control group.The patients in the con-trol group were treated with calcium hydroxide pulpotomy,and the patients in the observation group were treated with bioceramic iroot BP plus pulpotomy.The success rate,pain,dentin bridge formation rate,tooth discoloration rate and gingival health status of the two groups were compared.Results The repair success rates in the observation group were higher that these in the control group at 1(χ^(2)=4.286,P=0.038),3(χ^(2)=5.455,P=0.019),6(χ^(2)=6.405,P=0.015),and 12(χ^(2)=5.455,P=0.019)months.The visual analogue scale(VAS)scores of patients in the observation group at 1(t=4.292,P=0.001),3(t=3.349,P=0.001)and 6(t=8.876,P=0.001)months were lower than these in the control group.Compared to the control group,the observation had higher dentin bridge formation rates at each time point(χ^(2)=4.286,5.554,5.079,6.667;P=0.038,0.018,0.024,0.010 for 1,3,6,12 months respectively).The observation group had lower tooth discoloration rates at 1(χ^(2)=4.286,P=0.028),3(χ^(2)=4.043,P=0.044),6(χ^(2)=5.455,P=0.020),12(χ^(2)=5.963,P=0.015)months compared with the control group.The gingival index(GI)and sulcus bleeding index(SBI)scores of the observation group at 1(t=4.287,26.847,P<0.001),3(t=4.899,9.859,P<0.001),6(t=22.045,8.660,P<0.001),and 12(t=15.191,20.176,P<0.001)months were lower than these of the control group.Conclusion The use of bioceramic iroot BP plus for pulp capping during pulpotomy for crown fracture caused by dental trauma in children has a high rate of dentin bridge formation,good gingival health and significant therapeutic effect.
