炎症性肠病患者精神状况与病情及睡眠质量相关分析

摘　　要：目的探究炎症性肠病(IBD)患者精神状况与疾病活动、睡眠质量间的相互关系。方法采用线下发放问卷方法对2021年3月1日至6月30日于郑州大学第二附属医院住院已确诊IBD患者进行问卷调查并收集患者基本信息,分析影响患者焦虑、抑郁发生的因素。结果溃疡性结肠炎患者46例,合并焦虑26例(56.5%),合并抑郁20例(43.5%);克罗恩病患者81例,合并焦虑45例(55.6%),合并抑郁34例(42.0%)。疾病活动、复发>3次是溃疡性结肠炎患者焦虑发生的独立影响因素(P<0.05);肠道症状、全身症状、疾病活动、复发>3次、有经济负担是克罗恩病患者焦虑发生的独立影响因素(P<0.05);而情感问题是克罗恩病患者抑郁发生的独立影响因素(P<0.05),此外无独立影响溃疡性结肠炎患者抑郁发生的因素。结论溃疡性结肠炎、克罗恩病患者均存在睡眠问题,病情活动是两者焦虑发生的独立影响因素,情感问题是克罗恩病患者抑郁发生的独立影响因素。Objective To investigate the correlation between mental condition and disease activity and sleep quality in patients with inflammatory bowel disease(IBD).Methods The face-to-face questionnaire distribution method was adopted to conduct a questionnaire survey on the diagnosed patients with IBD hospitalized in the Second Affiliated Hospital of Zhengzhou University from March 1 to June 30,2021,and the basic information of all patients was collected.The factors impacting the occurrence of anxiety and depression were analyzed.Results There were 46 patients with ulcerative colitis,in which 26 patients(accounting for 56.5%)were complicated with anxiety and 20 patients(accounting for 43.5%)were complicated with depression.There were 81 patients with Crohn’s disease,in which 45 patients(accounting for 55.6%)were complicated with anxiety and 34 patients(accounting for 42.0%)were complicated with depression.Disease attack and relapse more than 3 times(P<0.05)are the independent impacting factors of anxiety in patients with ulcerative colitis.Intestinal symptoms,systemic symptoms,disease attack and relapse more than 3 times and financial burden(P<0.05)are the independent impacting factors of anxiety in patients with Crohn’s disease.However,emotional problems(P<0.05)are the independent impacting factor of depression in patients with Crohn’s disease,and there is no independent factor of depression in patients with ulcerative colitis.Conclusion Patients with ulcerative colitis and Crohn’s disease all have sleep problems,and disease attack is an independent impacting factor of both anxiety complications,while emotional problems are an independent impacting factor of depression complications in patients with Crohn’s disease.
