摘　　要：目的探究Mohs外科手术应用于头颈颌面部皮肤恶性肿瘤患者美学修复的临床效果。方法选取2019年1月至2021年2月广西壮族自治区皮肤病医院收治的74例头颈颌面部皮肤恶性肿瘤患者,通过随机数字表法分为对照组(n=37)和观察组(n=37),对照组接受传统手术切除,观察组接受Mohs外科手术治疗。比较两组手术时间、创面最长径、切口愈合时间;采用外观满意度调查问卷评估患者对美学修复满意度;随访1年,记录并比较两组患者复发、瘢痕形成、患者不良心理情况。结果观察组手术时间长于对照组,但创面最长径、切口愈合时间短于对照组,且观察组患者对美学修复总满意度高于对照组,差异有统计学意义(P<0.05);观察组随访1年期间复发率、瘢痕形成率均低于对照组,医院焦虑抑郁量表(HADS)评分低于对照组,差异有统计学意义(P<0.05)。结论Mohs外科手术可以在确保切除头颈颌面部皮肤恶性肿瘤时,尽可能保留正常组织,利于创面修复愈合,提高患者美学满意度。Objective To investigate the clinical effect on aesthetic repair of Mohs surgery in patients with malignant tumors of head,neck and maxillofacial skin.Methods A total of 74 patients with malignant tumors of head,neck and maxillofacial skin admitted to and treated in the Dermatology Hospital of Guangxi Zhuang Autonomous Region from January 2019 to February 2021 were selected and divided into the control group(n=37)and the observation group(n=37)using the random number table method.The control group was treated with conventional surgical resection,while the observation group was treated with Mohs surgical treatment.The operation time,the longest wound diameter and wound healing time were compared between the two groups.The patients’satisfaction with aesthetic repair was evaluated by appearance satisfaction questionnaire.The patients were followed up for 1 year,and the relapse,scar formation and adverse psychological conditions of the two groups of patients were recorded and compared.Results The operation time in the observation group was longer than that in the control.group,but the longest wound diameter and wound healing time in the observation group were shorter than those in the control group,and the total satisfaction of the patients in the observation group on aesthetic repair was higher than that in the control group,with statistically significant differences(P<0.05).The relapse rate and scar formation rate in the observation group were.both lower than those in the control group,and the score of hospital anxiety and depression scales(HADS)was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion Mohs surgery can ensure that in the removal of malignant tumors of head,neck and maxillofacial skin,normal tissues can be preserved as much as possible,thus facilitating wound repair and healing,and improving patients aesthetic satisfaction.
