摘　　要：目的 :通过临床风险主动识别与监控系统的构建与应用,树立护士风险管理的系统性思维,提升主动识别和规避临床风险的能力,保障患者安全。方法 :研发临床风险主动识别与监控系统,依托医院护理管理信息系统平台,构建风险识别与监控系统模块并应用于临床。结果 :2021年6月—9月,该系统识别临床潜在风险事件总计282例,其中患者受伤风险7例(2.48%),患者就医体验风险12例(4.26%),医疗风险35例(12.41%),员工受伤风险5例(1.77%),环境风险26例(9.22%),公共卫生事件风险177例(62.77%),其他风险20例(7.09%),潜在风险规避率为99.65%。结论 :早期、主动风险识别机制的建立是控制不良事件发生的重要途径,临床风险主动识别与监控系统的应用可提升临床对潜在风险的主动识别意识,为患者安全提供保障。Objective:Through the construction and application of the clinical risk active identifcation and monitoring system,we aim to promote nurses to establish systematic thinking of risk management,to actively identify and avoid clinical risks,and to ensure patient safety.Methods:Based on the hospital nursing management information system platform,the risk identifcation and monitoring system module was constructed and applied in clinical practice.Results:From June 2021to September 2021,the system identifed a total of 282 potential clinical risk events,including 7 patients with risk of injury(2.48%),12 patients with risk of medical examination(4.26%),35 patients with risk of medical treatment(12.41%),and 5employees with risk of injury(1.77%).There were 26 cases of environmental risk(9.22%),176 cases of public health event risk(62.77%),and 20 cases of other risk(7.09%).The incidence of avoiding potential risk was 99.65%.Conclusion:The establishment of early and active risk identifcation mechanism is an important way to control the occurrence of adverse events.The application of clinical risk active identifcation and monitoring system can improve the awareness of clinical active identifcation of potential risks and provide guarantee for patient safety.
