摘　　要：随着人们饮食习惯与生活方式的改变,非酒精性脂肪性肝病(NAFLD)的发病率逐年上升,其发病机制和治疗已成为肝病领域的研究热点。维生素D(VD)不足或缺乏与NAFLD的发病密切相关,低水平血清25-羟基维生素D(25-OH-D)与肝组织脂肪变性、炎症、纤维化程度有关。VD对NAFLD的影响可能通过VD受体(VDR)介导细胞内信号转导,作用于相关基因,调节脂质及葡萄糖代谢、细胞的增殖、分化及凋亡,起到抗炎、抗氧化、抗纤维化等作用。补充VD可减轻肝组织的病理改变,改善NAFLD相关指标,有望成为未来重要的NAFLD治疗方式,但VD的治疗剂量和作用时间还有待于大规模、大样本的临床试验来验证。该文从VD缺乏与NAFLD的关系、VD与NAFLD发病机制的关系、VD在NAFLD治疗中的作用等方面进行综述,为NAFLD的防治提供新的思路。With the changes in people's living habits and diet,the incidence of non-alcoholic fatty liver disease(NAFLD)is increased yearly,and the pathogenesis and treatment of NAFLD have become hot spots in the field of liver disease.Vitamin D(VD)deficiency is closely related to the onset of NAFLD,and low serum 25-OH-D level is related to steatosis,inflammation,and fibrosis in liver tissue.The effect of VD on NAFLD is to mediate intracellular signal transduction through VD receptor(VDR),regulate related genes,participate in glucose and lipid metabolism,cell proliferation,differentiation and apoptosis,and play anti-inflammatory,anti-oxidant and anti-fibrotic effects.VD supplementation can reduce liver pathological changes and improve NAFLD-related indicators,and is expected to become one of the important treatments for NAFLD.But its therapeutic dose and time of action still need to be verified by large-scale and large-sample clinical trials.This article reviews the relationship between VD deficiency and NAFLD,the relationship between VD and pathogenesis of NAFLD,and the role of VD in treatment of NAFLD,so to provide new ideas for the prevention and treatment of NAFLD.
