摘　　要：目的分析责任血管含椎-基底动脉压迫对面肌痉挛(HFS)病人微血管减压术疗效的影响。方法回顾性分析我院2017年1月—2019年12月收治的102例HFS病人临床资料,根据是否包含椎-基底动脉压迫分为对照组(小动脉压迫)68例、观察组(椎-基底动脉压迫)34例。两组病人均实施微血管减压术治疗。比较两组临床疗效、延迟治愈率、复发率和并发症等指标。结果观察组临床总有效率(76.47%)明显低于对照组(92.65%),对照组治愈病人延迟治愈率(14.89%)明显低于观察组(42.86%),观察组有效病人复发率(19.23%)明显高于对照组(3.17%),观察组并发症发生率(20.59%)明显高于对照组(4.41%),差异均有统计学意义( χ^(2)=3.978~5.003,P<0.05)。结论责任血管含有椎-基底动脉压迫可影响HFS微血管减压术治疗效果,延长治愈时间,增加复发率及并发症发生率。Objective To investigate the influence of offending vessels with vertebrobasilar artery compression on the therapeutic effect of microvascular decompression in patients with hemifacial spasm(HFS).Methods A retrospective analysis was performed for the clinical data of 102 HFS patients who were admitted to our hospital from January 2017 to December 2019,and according to the presence or absence of vertebrobasilar artery compression,they were divided into control group(compression of arterioles)with 68 patients and observation group(vertebrobasilar artery compression)with 34 patients.Microvascular decompression was performed for both groups,and the two groups were compared in terms of the indices such as clinical outcome,delayed cure rate,recurrence rate,and complications.Results The observation group had a significantly lower overall response rate than the control group(76.47%vs 92.65%),and the control group had a significantly lower delayed cure rate than the observation group(14.89%vs 42.86%);compared with the control group,the observation group had significantly higher recurrence rate(19.23%vs 3.17%)and incidence rate of complications(20.59%vs 4.41%),the difference between the two groups was statistically significant( χ^(2)=3.978-5.003,all P<0.05).Conclusion Offending vessels with vertebrobasilar artery compression may affect the therapeutic effect of microvascular decompression for HFS,prolong cure time,and increase recurrence rate and incidence rate of complications.
