星状玻璃体变性病人白内障术后视觉质量临床观察

摘　　要：目的探讨星状玻璃体变性(AH)对白内障术后病人视觉质量的影响。方法随机选取40例行双眼白内障手术治疗、伴有单眼AH病人作为研究对象,以白内障伴有AH眼为AH组,白内障不伴有AH眼为非AH组。检测两组病人术前与术后3个月的裸眼视力(UCVA)和最佳矫正视力(BCVA),UCVA和BCVA均换算为logMAR视力进行分析;术后3个月双眼均行4mm瞳孔下双通道客观视觉质量分析系统(OQAS^(TM)Ⅱ)检查,参数包括调制传递函数截止频率(MTFcutoff)、客观散射指数(OSI)、斯特列尔比(SR)、不同对比度下的视力(OV100%、OV20%、OV9%);并对所有病人进行主观视觉功能问卷调查评分。分析两组组间及组内手术前后各指标的变化。结果AH组与非AH组手术前、手术后UCVA及BCVA比较,差异无统计学意义(P>0.05);两组UCVA及BCVA术前术后差值比较,差异也无统计学意义(P>0.05)。组内比较,两组术后UCVA、BCVA均较术前提高,差异均有统计学意义(t=16.757~22.916,P<0.01)。两组MTFcutoff、OSI、SR、OV100%、OV20%及OV9%值比较,差异均有统计学意义(t=-9.592~4.003,P<0.01);两组病人近视力、中视力和远视力满意度比较,差异均无统计学意义(P>0.05)。AH组和非AH组发生飞蚊症的比例分别为15.0%(6眼)和7.5%(3眼),闪光感的比例分别为5.0%(2眼)和0(0眼),差异均无统计学意义(P>0.05)。结论AH对白内障病人术后的视力和主观感受无明显影响,但降低了客观视觉质量,增加了散射和衍射。Objective To investigate the influence of asteroid hyalosis(AH)on visual quality after cataract surgery.Methods A total of 40 patients with unilateral AH who underwent binocular cataract surgery were randomly selected as research subjects;the eyes with cataract and AH were selected as AH group and the eyes with cataract and without AH were selected as non-AH group.Uncorrected visual acuity(UCVA)and best corrected visual acuity(BCVA)were measured before surgery and at 3 months after surgery,and UCVA and BCVA were converted into logMAR visual acuity for analysis.Optical quality measurements for a 4 mm pupil were performed using OQAS^(TM) Ⅱat 3 months after surgery,and the parameters included MTF cut-off frequency,objective scattering index(OSI),Strehl ratio(SR),and visual acuity under different contrasts(OV100%,OV20%,and OV9%).All patients were assessed by a subjective visual function questionnaire,and the changes in each index after surgery were compared between groups and within each group.Results There were no significant differences between the AH group and the non-AH group in UCVA and BCVA before and after surgery(P>0.05),and there were also no significant differences between the two groups in the changes in UCVA and BCVA after surgery(P>0.05).Intra-group comparison showed that both groups had significant increases in UCVA and BCVA after surgery(t=16.757-22.916,P<0.01).There were significant differences between the two groups in MTF cut-off,OSI,SR,OV100%,OV20%,and OV9%(t=-9.592 to 4.003,P<0.01),and there was no significant difference in the degree of satisfaction with near vision,middle vision,and far vision between the two groups(P>0.05).There were no significant differences between the AH group and the non-AH group in the proportion of patients with floaters(15.0%(6 eyes)vs 7.5%(3 eyes),P>0.05)or flashing sensation(5.0%(2 eyes)vs 0(0 eyes),P>0.05).Conclusion AH has no significant influence on visual acuity and subjective perception after cataract surgery,but it reduces objective visual qua-lity and increas
