摘  要:目的:探讨CBCT在婴幼儿小下颌畸形下颌骨牵引成骨术(MDO)疗效评估中的临床应用价值。方法:对19例婴幼儿小下颌畸形伴阻塞性睡眠呼吸暂停综合征(OSAS)患儿于MDO术前、后行颌面部CBCT扫描,测量下颌骨左右径(A线)、前后径(B线)和下颌骨体夹角(α),以及上气道舌咽区最狭窄层面横截面积(CSA)、矢状径(AP)和横径(LR),并行统计学分析。结果:术后下颌骨A线、B线和舌咽区上气道AP、CSA均比术前显著增大,下颌骨α、A线/B线和舌咽区上气道LR/AP均比术前明显变小(均P<0.05);术后CSA与A线、B线、A线/B线及α均无明显相关性(均P>0.05)。术前α为钝角18例(94.7%),锐角1例(5.3%);术后α为钝角1例(5.3%)。结论:下颌骨和上气道的CBCT测量可为婴幼儿小下颌畸形MDO矫治疗效评估提供新的思路和方法。Objective:To investigate the clinical application value of CBCT for efficacy evaluation of mandibular distraction osteogenesis(MDO)to infants with micrognathia.Methods:CBCT scanning was performed on 19 infants with micrognathia and obstructive sleep apnea-hypopnea syndrome(OSAS)before and after MDO.The left-right diameter(line A),anterior-posterior diameter(line B),angle of corpus mandibulae(α),and the cross-sectional area(CSA),Sagittal diameter(AP)and transverse diameter(LR)of the narrowest level of glossopharyngeum in the upper airway were measured and analyzed.Results:The dimensions of line A and line B of mandible,AP dimension and CSA of glossopharyngeum in the upper airway increased significanty than before MDO,whileα,the ratio of line A to line B and the ratio of LR to AP decreased significantly than before MDO(all P<0.05).There was no significant correlation between CSA and line A,line B,ratio of line A to line B,α(all P>0.05).Before MDO,αwas obtuse angle in 18 cases(94.7%)and acute angle in 1 case(5.3%),meanwhile,after MDO,αwas obtuse angle in 1 case(5.3%).Conclusions:CBCT measurements of the mandible and upper airway can provide a new idea and method for evaluating the clinical efficacy of micrognathia in infants.
