摘 要:目的测量大前庭水管综合征(large vestibular aqueduct syndrome,LVAS)患者耳蜗的径线,探讨其在人工耳蜗植入术前评估的意义。方法收集符合入选标准的40例大前庭水管综合征患者(LVAS组)和40例内耳结构正常的先天性感音神经性聋患者(对照组)的颞骨高分辨率CT资料,通过多平面重组,测量耳蜗各径线数据,包括长径A、宽径B、高度H以及耳蜗蜗管长度L,比较两组结果。结果两组组内男女之间耳蜗各径线存在差异,女性各径线均小于男性(P<0.05)。LVAS患者的耳蜗长径A为8.88±0.43 mm、宽径B为6.57±0.43 mm、高度H为2.99±0.25 mm、蜗管长度L为33.94±1.99 mm;对照组分别为长径A 9.27±0.52 mm、宽径B 6.99±0.41 mm、高度H 3.04±0.34 mm、耳蜗蜗管长度L 35.79±2.18 mm。LVAS组长径A、宽径B、蜗管长度L较对照组小,差异有统计学意义(P<0.05),两组间耳蜗高度H差异无统计学意义(P>0.05)。结论大前庭水管综合征患者耳蜗长径A、宽径B及蜗管长度L比内耳结构正常的先天性感音神经性聋患者小,CI术前测量耳蜗各径线,可以为植入电极的个性化选择及手术提供参考。Objective To summarize and analyze cochlea dimensions data in patients with large vestibular aqueduct syndrome and to explore its effects and significance in preoperative evaluation of cochlear implant.Methods The clinical data of 40 patients with large vestibular aqueduct syndrome(LVAS)and 40 patients with congenital sensorineural hearing loss with normal inner ear structure were collected.Cochlea dimensions were measured by multiplanar reconstruction of temporal bone HRCT,including length diameter A,width diameter B,height H and cochlear duct length L,compared and analyzed the data between groups were analyzed and compared.Results There were differences in the diametors of the cochlea between male and female groups from the two different disease groups.The cochlea measurement data of the LVAS group were length diameter A(8.88±0.43)mm,width diameter B(6.57±0.43)mm,height H(2.99±0.25)mm,and cochlear duct length L(33.94±1.99)mm.The cochlea measurement data of the congenital sensorineural hearing loss with normal inner ear structure group were length diameter A(9.27±0.52)mm,width diameter B(6.99±0.41)mm,height H(3.04±0.34)mm,and cochlear duct length L(35.79±2.18)mm.The length diameter A,width diameter B and cochlear duct length L of LVAS group were smaller than control group,the difference was statistically significant(P<0.05).But for height H there was no significant difference between the two groups(P>0.05).Conclusion Cochlea length A,width B and cochlear duct length L of patients with LVAS are smaller than those with congenital sensorineural hearing loss with normal inner ear structure.For LVAS patients who are considering cochlear implantation,preoperative measurement and analysis of cochlea dimensions can provide objective evidence to select electrode with suitable length and provide guidance for surgery.
