摘 要:目的比较宫颈癌术后七野固定调强放疗(IMRT)与容积旋转调强放疗(VMAT)计划的差异。方法选取2018年1月至2019年12月在本院进行放疗的29例宫颈癌患者,分别制作IMRT和VMAT两种放疗计划,比较两种计划的计划靶体积(PTV)及股骨头、直肠、膀胱、小肠、骨盆等危及器官(OAR)的剂量学参数、机器跳数(MU)、治疗时间的差异。结果与IMRT计划比较,VMAT计划的D_(max)、D_(2)、V_(110%)更低,V_(100%)更高,均匀性指数(HI)、适形性指数(CI)更优(P<0.05)。VMAT计划的双侧股骨头V 50、D_(max)、D_(mean),直肠V_(30)、V_(40)、V_(45)、D_(min)、D_(max)、D_(mean),膀胱V_(20)、V_(30)、V_(40)、D_(mean),小肠V_(20)、V_(30)、D_(max),骨盆V_(20)、V_(30)、V_(40)、V 50、D_(mean)均低于IMRT计划(P<0.05)。VMAT计划的MU和治疗时间较IMRT计划均明显减少(P<0.05)。结论VMAT计划剂量学参数优于IMRT计划,且MU和治疗时间明显减少,提高了设备使用率。Objective To compare the dosimetric difference between the seven-fixed-field intensity-modulated radiotherapy(IMRT)and volumetric modulated arc therapy(VMAT)plan plan for the postoperative cervical cancer.Methods A total of 29 cervical cancer patients who received radiotherapy in this hospital from January 2018 to December 2019 were selected for this study.VMAT and IMRT plans were generated respectively.The plan target volume(PTV),dosimetry parameters of the organs at risk(OAR)such as femoral head,rectum,bladder,small intestine,and pelvis,monitor units(MU)and treatment time were compared between the two plans.Results Compared with those of the IMRT plan,the D_(max),D_(2)and V_(110%)of the VMAT plan were lower,the V_(100%)was higher,and the homogeneity index(HI)and conformability index(CI)were better(P<0.05).Compared with those of the IMRT plan,the V 50,D_(max)and D_(mean)of the femoral head,the V_(30),V_(40),V_(45),D_(min),D_(max)and D_(mean)of the rectum,the V_(20),V_(30),V_(40)and D_(mean)of the bladder,the V_(20),V_(30)and D_(max)of bowel and the V_(20),V_(30),V_(40),V 50 and D_(mean)of the pelvis in the VMAT plan were lower(P<0.05).Compared with those of the IMRT plan,the MU and treatment time of the VMAT plan significantly reduced(P<0.05).Conclusion The dosimetry parameters of the VMAT plan are better than those of the IMRT plan,and the MU and treatment time are obviously lower,which improves the equipment utilization rate.
