纯钛支架义齿修复治疗牙列缺损伴牙周病的效果

摘　　要：目的探讨纯钛支架义齿修复治疗牙列缺损伴牙周病(PD)的效果。方法选取濮阳市人民医院收治的67例牙列缺损伴PD患者为研究对象,根据随机数字表法分为对照组(33例)、观察组(34例)。对照组接受钴铬合金支架义齿修复,观察组接受纯钛支架义齿修复。比较修复效果及修复前后牙周状态相关指标、龈沟液细胞因子水平、牙本质敏感性、咀嚼功能、美观程度、神经递质水平。结果观察组总有效率高于对照组(P<0.05)。修复后,观察组牙龈指数(GI)、龈沟出血指数(SBI)、牙周袋探诊深度(PD)、牙菌斑指数(PLI)、临床附着水平(CAL)、牙齿松动度(TM)低于对照组,Vaspin、白细胞介素-1β(IL-1β)、基质金属蛋白酶-3(MMP-3)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、视觉模拟评分法(VAS)得分均低于对照组,咀嚼效率、口腔美观、口腔清洁、牙龈健康、整体美观评分、多巴胺(DA)、去甲肾上腺素(NE)、5羟色胺(5-HT)水平均高于对照组(P<0.05)。结论纯钛支架义齿修复可改善牙列缺损伴PD患者的牙周状态,控制龈沟液炎症细胞因子释放,改善牙本质敏感性,提升咀嚼功能,提高美观度,调节脑神经递质水平,具有临床应用价值。Objective To investigate the effect of pure titanium stented dentures in the treatment of dentition defect with periodontal disease(PD).Methods A total of 67 patients with dentition defect and PD who were treated in Puyang People’s Hospital were selected and divided into control group(33 cases)and observation group(34 cases)by random number table method.The control group received cobalt-chromium alloy stent denture repair,and the observation group received pure titanium stented denture repair.Restoration effect,periodontal status-related indicators,gingival crevicular fluid cytokine levels,dentin sensitivity,masticatory function,aesthetic level and neurotransmitter level before and after restoration were compared.Results The total effective rate of observation group was higher than that of control group(P<0.05).After repair the gingival index(GI),gingival sulcus bleeding index(SBI),probing depth of periodontal pocket(PD),plaque index(PLI),clinical attachment level(CAL),tooth mobility(TM)of observation group were lower than those of control group,the levels of Vaspin,interleukin-1β(IL-1β),matrix metalloproteinase-3(MMP-3),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and visual analogue scale(VAS)score were lower than those of control group,and the chewing efficiency,oral appearance,oral hygiene,gum health,overall appearance score,dopamine(DA),norepinephrine(NE),serotonin(5-HT)levels were higher than those of control group(P<0.05).Conclusion Pure titanium stented denture can improve periodontal status of dentition defect with PD patients,control the release of inflammatory cytokines in gingival crevicular fluid,improve dentin sensitivity,improve masticatory function,improve aesthetics,and regulate the level of brain neurotransmitters,and with clinical application value.
